Depth distributions of positron-emitting nuclei in PMMA phantoms are calculated within a Monte Carlo model for Heavy-Ion Therapy (MCHIT) based on the GEANT4 toolkit (version 8.0). The calculated total production rates of $^{11}$C, $^{10}$C and $^{15}$O nuclei are compared with experimental data and with corresponding results of the FLUKA and POSGEN codes. The distributions of e$^+$ annihilation points are obtained by simulating radioactive decay of unstable nuclei and transporting positrons in surrounding medium. A finite spatial resolution of the Positron Emission Tomography (PET) is taken into account in a simplified way. Depth distributions of $β^+$-activity as seen by a PET scanner are calculated and compared to available data for PMMA phantoms. The calculated $β^+$-activity profiles are in good agreement with PET data for proton and $^{12}$C beams at energies suitable for particle therapy. The MCHIT capability to predict the $β^+$-activity and dose distributions in tissue-like materials of different chemical composition is demonstrated.
Single-photon emission computed tomography (SPECT) provides a mechanism to estimate regional isotope uptake in lesions and at-risk organs after administration of α-particle-emitting radiopharmaceutical therapies (α-RPTs). However, this estimation task is challenging due to the complex emission spectra, the very low number of detected counts, the impact of stray-radiation-related noise at these low counts, and the multiple image-degrading processes in SPECT. The conventional reconstruction-based quantification methods are observed to be erroneous for α-RPT SPECT. To address these challenges, we…
We study the spatial distributions of $β^+$-activity produced by therapeutic beams of $^3$He and $^{12}$C ions in various tissue-like materials. The calculations were performed within a Monte Carlo model for Heavy-Ion Therapy (MCHIT) based on the GEANT4 toolkit. The contributions from $^{10,11}$C, $^{13}$N, $^{14,15}$O, $^{17,18}$F and $^{30}$P positron-emitting nuclei were calculated and compared with experimental data obtained during and after irradiation. Positron emitting nuclei are created by $^{12}$C beam in fragmentation reactions of projectile and target nuclei. This leads to a $β^+$-a…
Carbon ion therapy have the ability to overcome the limitation of convertional radiotherapy due to its most energy deposition in selective depth, usually called Bragg peak, which results in increased biological effectiness. During carbon ion therapy, lots positron emitters such as $^{11}$C, $^{15}$O, $^{10}$C are generated in irradiated tissues by nuclear reactions. Immediately after patient irradiation, PET scanners can be used to measure the spatial distribution of positron emitters, which can track the carbon beam to the tissue. In this study, we designed and evaluated an dual-plate in-room…
We study the propagation of nucleons and nuclei in tissue-like media within a Monte Carlo Model for Heavy-ion Therapy (MCHIT) based on the GEANT4 toolkit (version 8.2). The model takes into account fragmentation of projectile nuclei and secondary interactions of produced nuclear fragments. Model predictions are validated with available experimental data obtained for water and PMMA phantoms irradiated by monoenergetic carbon-ion beams. The MCHIT model describes well (1) the depth-dose distributions in water and PMMA, (2) the doses measured for fragments of certain charge, (3) the distributions …

Dose verification based on proton-induced positron emitters is a promising quality assurance tool and may leverage the strength of artificial intelligence. To move a step closer towards practical application, the sensitivity analysis of two factors needs to be performed: biological washout and depth selection. selection. A bi-directional recurrent neural network (RNN) model was developed. The training dataset was generated based upon a CT image-based phantom (abdomen region) and multiple beam energies/pathways, using Monte-Carlo simulation (1 mm spatial resolution, no biological washout). For …
Background and purpose: In-beam Positron Emission Tomography (PET) is one of the modalities that can be used for in-vivo non-invasive treatment monitoring in proton therapy. PET distributions obtained during various treatment sessions can be compared in order to identify regions that have anatomical changes. The purpose of this work is to test and compare different analysis methods in the context of inter-fractional PET image comparison for proton treatment verification. Methods: For our study we used the FLUKA Monte Carlo code and artificially generated CT scans to simulate in-beam PET distri…
$Objective$. Obtaining the intrinsic dose distributions in particle therapy is a challenging problem that needs to be addressed by imaging algorithms to take advantage of secondary particle detectors. In this work, we investigate the utility of deep learning methods for achieving direct mapping from detector data to the intrinsic dose distribution. $Approach$. We performed Monte Carlo simulations using GATE/Geant4 10.4 simulation toolkits to generate a dataset using human CT phantom irradiated with high-energy protons and imaged with compact in-beam PET for realistic beam delivery in a single-…
PET is an established medical diagnostic imaging method. Continuous improvements are aimed at refining image reconstruction, reducing the amount of radioactive tracer and combining with targeted therapy. TOF-PET provides the localization of the tracer through improved time resolution, nuclear physics may contribute to this goal via selection of radioactive nuclei emitting additional $γ$-rays. This additional radiation, when properly detected, localizes the decay of the tracer at the line of response determined by two detected 511 keV quanta. Selected candidates are presented. Some are particul…

Dynamic positron emission tomography (dPET) is currently a widely used medical imaging technique for the clinical diagnosis, staging and therapy guidance of all kinds of human cancers. Higher temporal imaging resolution for the early stage of radiotracer metabolism is desired; however, in this case, the reconstructed images with short frame durations always suffer from a limited image signal-to-noise ratio (SNR) and unsatisfactory image spatial resolution. The appearance of uEXPLORER (United Imaging Healthcare, Inc.) with higher PET imaging sensitivity and resolution may help solving this prob…
Proton and carbon ion therapy is an emerging technique used for the treatment of solid cancers. The monitoring of the dose delivered during such treatments and the on-line knowledge of the Bragg peak position is still a matter of research. A possible technique exploits the collinear $511\ \kilo\electronvolt$ photons produced by positrons annihilation from $β^+$ emitters created by the beam. This paper reports rate measurements of the $511\ \kilo\electronvolt$ photons emitted after the interactions of a $80\ \mega\electronvolt / u$ fully stripped carbon ion beam at the Laboratori Nazionali del …
Modern PET systems reach a spatial resolution of 3-10 mm. A disadvantage of this technique is the diffusion of the positron before its decay with a typical range of ca. 1 mm (depending on its energy). This motion and Compton scattering of the 511 keV photons within the patient limit the performance of PET. We present a nuclear medical imaging technique, able to reach submillimeter spatial resolution in 3 dimensions with a reduced activity application compared to conventional PET. This 'gamma-PET' technique draws on specific positron sources simultaneously emitting an additional photon with the…
Objective: The aim of this work is to investigate the feasibility of the Jagiellonian Positron Emission Tomography (J-PET) scanner for intra-treatment proton beam range monitoring. Approach: The Monte Carlo simulation studies with GATE and PET image reconstruction with CASToR were performed in order to compare six J-PET scanner geometries (three dual-heads and three cylindrical). We simulated proton irradiation of a PMMA phantom with a Single Pencil Beam (SPB) and Spread-Out Bragg Peak (SOBP) of various ranges. The sensitivity and precision of each scanner were calculated, and considering the …

In this paper, we review physics- and data-driven reconstruction techniques for simultaneous positron emission tomography (PET) / magnetic resonance imaging (MRI) systems, which have significant advantages for clinical imaging of cancer, neurological disorders, and heart disease. These reconstruction approaches utilize priors, either structural or statistical, together with a physics-based description of the PET system response. However, due to the nested representation of the forward problem, direct PET/MRI reconstruction is a nonlinear problem. We elucidate how a multi-faceted approach accom…
Targeted Alpha Therapy potentially offers a more specific action in killing tumor cells and less damage to neighbouring normal cells compared to $β$-emitters. $^{225}$Ac along with three other $α$-emitting daughter nuclei $^{221}$Fr, $^{217}$At and $^{213}$Po have significant potency for clinical use. Monte carlo simulation has been carried out using MONC code for production routes of $^{225}$Ac for its possible generation using proton accelerators with Thorium-232 and Radium-226 targets. All the calculations has been carried out using global model parameters without adjusting for individual t…
We report on an hybrid in-beam PET and prompt-gamma Compton imaging system aimed at quasi real-time ion-range verification in proton-therapy treatments. Proof-of-concept experiments were carried out at the radiobiology beam line of the CNA cyclotron facility using a set of two synchronous Compton imagers and different target materials. The time structure of the 18~MeV proton beam was shaped with a series of beam-on and beam-off intervals, thereby mimicking a pulsed proton beam on a long time scale. During beam-on intervals, Compton imaging was performed utilizing the high energy $γ$-rays promp…
