Deep learning for medical imaging is hampered by task-specific models that lack generalizability and prognostic capabilities, while existing 'universal' approaches suffer from simplistic conditioning and poor medical semantic understanding. To address these limitations, we introduce DuPLUS, a deep learning framework for efficient multi-modal medical image analysis. DuPLUS introduces a novel vision-language framework that leverages hierarchical semantic prompts for fine-grained control over the analysis task, a capability absent in prior universal models. To enable extensibility to other medical tasks, it includes a hierarchical, text-controlled architecture driven by a unique dual-prompt mechanism. For segmentation, DuPLUS is able to generalize across three imaging modalities, ten different anatomically various medical datasets, encompassing more than 30 organs and tumor types. It outperforms the state-of-the-art task specific and universal models on 8 out of 10 datasets. We demonstrate extensibility of its text-controlled architecture by seamless integration of electronic health record (EHR) data for prognosis prediction, and on a head and neck cancer dataset, DuPLUS achieved a Concordance Index (CI) of 0.69. Parameter-efficient fine-tuning enables rapid adaptation to new tasks and modalities from varying centers, establishing DuPLUS as a versatile and clinically relevant solution for medical image analysis. Since the advent of U-Net, fully convolutional deep neural networks and its many variants have completely changed the modern landscape of deep learning based medical image segmentation. However, the over dependence of these methods on pixel level classification and regression has been identified early on as a problem. Especially when trained on medical databases with sparse available annotation, these methods are prone to generate segmentation artifacts such as fragmented structures, topological inconsistencies and islands of pixel. These artefacts are especially problematic in medical imaging…

Artificial intelligence (AI) techniques have significant potential to enable effective, robust and automated image phenotyping including identification of subtle patterns. AI-based detection searches the image space to find the regions of interest based on patterns and features. There is a spectrum of tumor histologies from benign to malignant that can be identified by AI-based classification approaches using image features. The extraction of minable information from images gives way to the field of radiomics and can be explored via explicit (handcrafted/engineered) and deep radiomics framewor…

Assessing the location and extent of lesions caused by chronic stroke is critical for medical diagnosis, surgical planning, and prognosis. In recent years, with the rapid development of 2D and 3D convolutional neural networks (CNN), the encoder-decoder structure has shown great potential in the field of medical image segmentation. However, the 2D CNN ignores the 3D information of medical images, while the 3D CNN suffers from high computational resource demands. This paper proposes a new architecture called dimension-fusion-UNet (D-UNet), which combines 2D and 3D convolution innovatively in the…

The novel Coronavirus disease, COVID-19, has rapidly and abruptly changed the world as we knew in 2020. It becomes the most unprecedent challenge to analytic epidemiology in general and signal processing theories in specific. Given its high contingency nature and adverse effects across the world, it is important to develop efficient processing/learning models to overcome this pandemic and be prepared for potential future ones. In this regard, medical imaging plays an important role for the management of COVID-19. Human-centered interpretation of medical images is, however, tedious and can be s…
Each woman living in the United States has about 1 in 8 chance of developing invasive breast cancer. The mitotic cell count is one of the most common tests to assess the aggressiveness or grade of breast cancer. In this prognosis, histopathology images must be examined by a pathologist using high-resolution microscopes to count the cells. Unfortunately, this can be an exhaustive task with poor reproducibility, especially for non-experts. Deep learning networks have recently been adapted to medical applications which are able to automatically localize these regions of interest. However, these r…

The identification and quantification of markers in medical images is critical for diagnosis, prognosis and management of patients in clinical practice. Supervised- or weakly supervised training enables the detection of findings that are known a priori. It does not scale well, and a priori definition limits the vocabulary of markers to known entities reducing the accuracy of diagnosis and prognosis. Here, we propose the identification of anomalies in large-scale medical imaging data using healthy examples as a reference. We detect and categorize candidates for anomaly findings untypical for th…

Cox proportional hazard model (CPH) is commonly used in clinical research for survival analysis. In quantitative medical imaging (radiomics) studies, CPH plays an important role in feature reduction and modeling. However, the underlying linear assumption of CPH model limits the prognostic performance. In addition, the multicollinearity of radiomic features and multiple testing problem further impedes the CPH models performance. In this work, using transfer learning, a convolutional neural network (CNN) based survival model was built and tested on preoperative CT images of resectable Pancreatic…

Motivation: Medical image analysis involves tasks to assist physicians in qualitative and quantitative analysis of lesions or anatomical structures, significantly improving the accuracy and reliability of diagnosis and prognosis. Traditionally, these tasks are finished by physicians or medical physicists and lead to two major problems: (i) low efficiency; (ii) biased by personal experience. In the past decade, many machine learning methods have been applied to accelerate and automate the image analysis process. Compared to the enormous deployments of supervised and unsupervised learning models…
