Automatic classification of diabetic retinopathy from retinal images has been widely studied using deep neural networks with impressive results. However, there is a clinical need for estimation of the uncertainty in the classifications, a shortcoming of modern neural networks. Recently, approximate Bayesian deep learning methods have been proposed for the task but the studies have only considered the binary referable/non-referable diabetic retinopathy classification applied to benchmark datasets. We present novel results by systematically investigating a clinical dataset and a clinically relevant 5-class classification scheme, in addition to benchmark datasets and the binary classification scheme. Moreover, we derive a connection between uncertainty measures and classifier risk, from which we develop a new uncertainty measure. We observe that the previously proposed entropy-based uncertainty measure generalizes to the clinical dataset on the binary classification scheme but not on the 5-class scheme, whereas our new uncertainty measure generalizes to the latter case.

Visual artefacts of early diabetic retinopathy in retinal fundus images are usually small in size, inconspicuous, and scattered all over retina. Detecting diabetic retinopathy requires physicians to look at the whole image and fixate on some specific regions to locate potential biomarkers of the disease. Therefore, getting inspiration from ophthalmologist, we propose to combine coarse-grained classifiers that detect discriminating features from the whole images, with a recent breed of fine-grained classifiers that discover and pay particular attention to pathologically significant regions. To …
Deep learning-based models are developed to automatically detect if a retina image is `referable' in diabetic retinopathy (DR) screening. However, their classification accuracy degrades as the input images distributionally shift from their training distribution. Further, even if the input is not a retina image, a standard DR classifier produces a high confident prediction that the image is `referable'. Our paper presents a Dirichlet Prior Network-based framework to address this issue. It utilizes an out-of-distribution (OOD) detector model and a DR classification model to improve generalizabil…

In this paper, we propose an explainable and interpretable diabetic retinopathy (ExplainDR) classification model based on neural-symbolic learning. To gain explainability, a highlevel symbolic representation should be considered in decision making. Specifically, we introduce a human-readable symbolic representation, which follows a taxonomy style of diabetic retinopathy characteristics related to eye health conditions to achieve explainability. We then include humanreadable features obtained from the symbolic representation in the disease prediction. Experimental results on a diabetic retinopa…

Diabetic Retinopathy (DR) is a severe complication of diabetes that can cause blindness. Although effective treatments exist (notably laser) to slow the progression of the disease and prevent blindness, the best treatment remains prevention through regular check-ups (at least once a year) with an ophthalmologist. Optical Coherence Tomography Angiography (OCTA) allows for the visualization of the retinal vascularization, and the choroid at the microvascular level in great detail. This allows doctors to diagnose DR with more precision. In recent years, algorithms for DR diagnosis have emerged al…
We proposed a deep learning method for interpretable diabetic retinopathy (DR) detection. The visual-interpretable feature of the proposed method is achieved by adding the regression activation map (RAM) after the global averaging pooling layer of the convolutional networks (CNN). With RAM, the proposed model can localize the discriminative regions of an retina image to show the specific region of interest in terms of its severity level. We believe this advantage of the proposed deep learning model is highly desired for DR detection because in practice, users are not only interested with high …

This paper investigates the problem of domain adaptation for diabetic retinopathy (DR) grading. We learn invariant target-domain features by defining a novel self-supervised task based on retinal vessel image reconstructions, inspired by medical domain knowledge. Then, a benchmark of current state-of-the-art unsupervised domain adaptation methods on the DR problem is provided. It can be shown that our approach outperforms existing domain adaption strategies. Furthermore, when utilizing entire training data in the target domain, we are able to compete with several state-of-the-art approaches in…

Diabetic retinopathy (DR) is one of the leading causes of blindness in the working-age population of developed countries, caused by a side effect of diabetes that reduces the blood supply to the retina. Deep neural networks have been widely used in automated systems for DR classification on eye fundus images. However, these models need a large number of annotated images. In the medical domain, annotations from experts are costly, tedious, and time-consuming; as a result, a limited number of annotated images are available. This paper presents a semi-supervised method that leverages unlabeled im…

People with diabetes are at risk of developing an eye disease called diabetic retinopathy (DR). This disease occurs when high blood glucose levels cause damage to blood vessels in the retina. Computer-aided DR diagnosis is a promising tool for early detection of DR and severity grading, due to the great success of deep learning. However, most current DR diagnosis systems do not achieve satisfactory performance or interpretability for ophthalmologists, due to the lack of training data with consistent and fine-grained annotations. To address this problem, we construct a large fine-grained annota…

There is an increasing number of medical use-cases where classification algorithms based on deep neural networks reach performance levels that are competitive with human medical experts. To alleviate the challenges of small dataset sizes, these systems often rely on pretraining. In this work, we aim to assess the broader implications of these approaches. For diabetic retinopathy grading as exemplary use case, we compare the impact of different training procedures including recently established self-supervised pretraining methods based on contrastive learning. To this end, we investigate differ…

Bayesian deep learning seeks to equip deep neural networks with the ability to precisely quantify their predictive uncertainty, and has promised to make deep learning more reliable for safety-critical real-world applications. Yet, existing Bayesian deep learning methods fall short of this promise; new methods continue to be evaluated on unrealistic test beds that do not reflect the complexities of downstream real-world tasks that would benefit most from reliable uncertainty quantification. We propose the RETINA Benchmark, a set of real-world tasks that accurately reflect such complexities and …

Knowledge distillation allows transferring knowledge from a pre-trained model to another. However, it suffers from limitations, and constraints related to the two models need to be architecturally similar. Knowledge distillation addresses some of the shortcomings associated with transfer learning by generalizing a complex model to a lighter model. However, some parts of the knowledge may not be distilled by knowledge distillation sufficiently. In this paper, a novel knowledge distillation approach using transfer learning is proposed. The proposed method transfers the entire knowledge of a mode…

Diabetic retinopathy (DR) is one of the most common eye conditions among diabetic patients. However, vision loss occurs primarily in the late stages of DR, and the symptoms of visual impairment, ranging from mild to severe, can vary greatly, adding to the burden of diagnosis and treatment in clinical practice. Deep learning methods based on retinal images have achieved remarkable success in automatic DR grading, but most of them neglect that the presence of diabetes usually affects both eyes, and ophthalmologists usually compare both eyes concurrently for DR diagnosis, leaving correlations bet…

Recently, deep learning techniques have been successfully applied for detection of diabetic retinopathy (DR). The main obstacle of applying deep learning techniques to detect most other retinal vascular diseases is the limited amount of data available. In this paper, we propose a transfer learning technique that aims to utilize the feature similarities for detecting retinal vascular diseases. However, commonly used techniques are deterministic and are therefore unable to provide any estimate of predictive uncertainty. Quantifying model uncertainty is crucial for reducing the risk of misdiagnosis. A reliable architecture should be well-calibrated to avoid over-confident predictions. Early diagnosis of diabetic retinopathy for treatment of the disease has been failing to reach diabetic people living in rural areas. Shortage of trained ophthalmologists, limited availability of healthcare centers, and expensiveness of diagnostic equipment are among the reasons. Although many deep learning-based automatic diagnosis of diabetic retinopathy techniques have been implemented in the literature, these methods still fail to provide a point-of-care diagnosis. This raises the need for an independent diagnostic of diabetic retinopathy that can be used by a non-expert. Recently the usag…

Automated detection of diabetic retinopathy is essential to tackle these problems. Early-stage detection of diabetic retinopathy is also very important for diagnosis, which ca…
Recently, diabetic retinopathy (DR) screening utilizing ultra-wide optical coherence tomography angiography (UW-OCTA) has been used in clinical practices to detect signs of early DR. However, developing a deep learning-based DR analysis system using UW-OCTA images is not trivial due to the difficulty of data collection and the absence of public datasets. By realistic constraints, a model trained on small datasets may obtain sub-par performance. Therefore, to help ophthalmologists be less confused about models' incorrect decisions, the models should be robust even in data scarcity settings. To …
