Patient similarity analysis is important in health care applications. It takes patient information such as their electronic medical records and genetic data as input and computes the pairwise similarity between patients. Procedures of typical a patient similarity study can be divided into several steps including data integration, similarity measurement, and neighborhood identification. And according to an analysis of patient similarity, doctors can easily find the most suitable treatments. There are many methods to analyze the similarity such as cluster analysis. And during machine learning become more and more popular, Using neural networks such as CNN is a new hot topic. This review summarizes representative methods used in each step and discusses applications of patient similarity networks especially in the context of precision medicine.
Predicting the health risks of patients using Electronic Health Records (EHR) has attracted considerable attention in recent years, especially with the development of deep learning techniques. Health risk refers to the probability of the occurrence of a specific health outcome for a specific patient. The predicted risks can be used to support decision-making by healthcare professionals. EHRs are structured patient journey data. Each patient journey contains a chronological set of clinical events, and within each clinical event, there is a set of clinical/medical activities. Due to variations o…
This study develops a pattern recognition method that identifies patterns based on their similarity and their association with the outcome of interest. The practical purpose of developing this pattern recognition method is to group patients, who are injured in transport accidents, in the early stages post-injury. This grouping is based on distinctive patterns in health service use within the first week post-injury. The groups also provide predictive information towards the total cost of medication process. As a result, the group of patients who have undesirable outcomes are identified as early…
Patient similarity assessment (PSA) is pivotal to evidence-based and personalized medicine, enabled by analyzing the increasingly available electronic health records (EHRs). However, machine learning approaches for PSA has to deal with inherent data deficiencies of EHRs, namely missing values, noise, and small sample sizes. In this work, an end-to-end discriminative learning framework, called SparGE, is proposed to address these data challenges of EHR for PSA. SparGE measures similarity by jointly sparse coding and graph embedding. First, we use low-rank constrained sparse coding to identify a…
The increase in availability of longitudinal electronic health record (EHR) data is leading to improved understanding of diseases and discovery of novel phenotypes. The majority of clustering algorithms focus only on patient trajectories, yet patients with similar trajectories may have different outcomes. Finding subgroups of patients with different trajectories and outcomes can guide future drug development and improve recruitment to clinical trials. We develop a recurrent neural network autoencoder to cluster EHR data using reconstruction, outcome, and clustering losses which can be weighted…

The vast amount of health data has been continuously collected for each patient, providing opportunities to support diverse healthcare predictive tasks such as seizure detection and hospitalization prediction. Existing models are mostly trained on other patients data and evaluated on new patients. Many of them might suffer from poor generalizability. One key reason can be overfitting due to the unique information related to patient identities and their data collection environments, referred to as patient covariates in the paper. These patient covariates usually do not contribute to predicting …

Most children infected with COVID-19 have no or mild symptoms and can recover automatically by themselves, but some pediatric COVID-19 patients need to be hospitalized or even to receive intensive medical care (e.g., invasive mechanical ventilation or cardiovascular support) to recover from the illnesses. Therefore, it is critical to predict the severe health risk that COVID-19 infection poses to children to provide precise and timely medical care for vulnerable pediatric COVID-19 patients. However, predicting the severe health risk for COVID-19 patients including children remains a significan…

Class imbalance can often degrade predictive performance of supervised learning algorithms. Balanced classes can be obtained by oversampling exact copies, with noise, or interpolation between nearest neighbours (as in traditional SMOTE methods). Oversampling tabular data using augmentation, as is typical in computer vision tasks, can be achieved with deep generative models. Deep generative models are effective data synthesisers due to their ability to capture complex underlying distributions. Synthetic data in healthcare can enhance interoperability between healthcare providers by ensuring pat…

Bedside monitors in Intensive Care Units (ICUs) frequently sound incorrectly, slowing response times and desensitising nurses to alarms (Chambrin, 2001), causing true alarms to be missed (Hug et al., 2011). We compare sliding window predictors with recurrent predictors to classify patient state-of-health from ICU multivariate time series; we report slightly improved performance for the RNN for three out of four targets.

Comorbid chronic conditions are common among people with type 2 diabetes. We developed an Artificial Intelligence algorithm, based on Reinforcement Learning (RL), for personalized diabetes and multi-morbidity management with strong potential to improve health outcomes relative to current clinical practice. In this paper, we modeled glycemia, blood pressure and cardiovascular disease (CVD) risk as health outcomes using a retrospective cohort of 16,665 patients with type 2 diabetes from New York University Langone Health ambulatory care electronic health records in 2009 to 2017. We trained a RL …
Studies suggest that one in three US adults use the Internet to diagnose or learn about a health concern. However, such access to health information online could exacerbate the disparities in health information availability and use. Health information seeking behavior (HISB) refers to the ways in which individuals seek information about their health, risks, illnesses, and health-protective behaviors. For patients engaging in searches for health information on digital media platforms, health literacy divides can be exacerbated both by their own lack of knowledge and by algorithmic recommendatio…
Can health entities collaboratively train deep learning models without sharing sensitive raw data? This paper proposes several configurations of a distributed deep learning method called SplitNN to facilitate such collaborations. SplitNN does not share raw data or model details with collaborating institutions. The proposed configurations of splitNN cater to practical settings of i) entities holding different modalities of patient data, ii) centralized and local health entities collaborating on multiple tasks and iii) learning without sharing labels. We compare performance and resource efficien…

Longitudinal patient data has the potential to improve clinical risk stratification models for disease. However, chronic diseases that progress slowly over time are often heterogeneous in their clinical presentation. Patients may progress through disease stages at varying rates. This leads to pathophysiological misalignment over time, making it difficult to consistently compare patients in a clinically meaningful way. Furthermore, patients present clinically for the first time at different stages of disease. This eliminates the possibility of simply aligning patients based on their initial pre…
Patient time series classification faces challenges in high degrees of dimensionality and missingness. In light of patient similarity theory, this study explores effective temporal feature engineering and reduction, missing value imputation, and change point detection methods that can afford similarity-based classification models with desirable accuracy enhancement. We select a piecewise aggregation approximation method to extract fine-grain temporal features and propose a minimalist method to impute missing values in temporal features. For dimensionality reduction, we adopt a gradient descent…

Patient similarity assessment, which identifies patients similar to a given patient, can help improve medical care. The assessment can be performed using Electronic Medical Records (EMRs). Patient similarity measurement requires converting heterogeneous EMRs into comparable formats to calculate their distance. While versatile document representation learning methods have been developed in recent years, it is still unclear how complex EMR data should be processed to create the most useful patient representations. This study presents a new data representation method for EMRs that takes the infor…
Widespread adoption of electronic health records (EHRs) has fueled the development of using machine learning to build prediction models for various clinical outcomes. This process is often constrained by having a relatively small number of patient records for training the model. We demonstrate that using patient representation schemes inspired from techniques in natural language processing can increase the accuracy of clinical prediction models by transferring information learned from the entire patient population to the task of training a specific model, where only a subset of the population …

Models have been proposed to extract temporal patterns from longitudinal electronic health records (EHR) for clinical predictive models. However, the common relations among patients (e.g., receiving the same medical treatments) were rarely considered. In this paper, we propose to learn patient similarity features as phenotypes from the aggregated patient-medical service matrix using non-negative matrix factorization. On real-world medical claim data, we show that the learned phenotypes are coherent within each group, and also explanatory and indicative of targeted diseases. We conducted experi…
Delirium is a common acute onset brain dysfunction in the emergency setting and is associated with higher mortality. It is difficult to detect and monitor since its presentations and risk factors can be different depending on the underlying medical condition of patients. In our study, we aimed to identify subtypes within the delirium population and build subgroup-specific predictive models to detect delirium using Medical Information Mart for Intensive Care IV (MIMIC-IV) data. We showed that clusters exist within the delirium population. Differences in feature importance were also observed for…
We show how to learn low-dimensional representations (embeddings) of patient visits from the corresponding electronic health record (EHR) where International Classification of Diseases (ICD) diagnosis codes are removed. We expect that these embeddings will be useful for the construction of predictive statistical models anticipated to drive personalized medicine and improve healthcare quality. These embeddings are learned using a deep neural network trained to predict ICD diagnosis categories. We show that our embeddings capture relevant clinical informations and can be used directly as input t…
As hospitals move towards automating and integrating their computing systems, more fine-grained hospital operations data are becoming available. These data include hospital architectural drawings, logs of interactions between patients and healthcare professionals, prescription data, procedures data, and data on patient admission, discharge, and transfers. This has opened up many fascinating avenues for healthcare-related prediction tasks for improving patient care. However, in order to leverage off-the-shelf machine learning software for these tasks, one needs to learn structured representatio…
Building models for health prediction based on Electronic Health Records (EHR) has become an active research area. EHR patient journey data consists of patient time-ordered clinical events/visits from patients. Most existing studies focus on modeling long-term dependencies between visits, without explicitly taking short-term correlations between consecutive visits into account, where irregular time intervals, incorporated as auxiliary information, are fed into health prediction models to capture latent progressive patterns of patient journeys. We present a novel deep neural network with four m…

Rare diseases are very difficult to identify among large number of other possible diagnoses. Better availability of patient data and improvement in machine learning algorithms empower us to tackle this problem computationally. In this paper, we target one such rare disease - cardiac amyloidosis. We aim to automate the process of identifying potential cardiac amyloidosis patients with the help of machine learning algorithms and also learn most predictive factors. With the help of experienced cardiologists, we prepared a gold standard with 73 positive (cardiac amyloidosis) and 197 negative insta…

In recent years, increasingly augmentation of health data, such as patient Electronic Health Records (EHR), are becoming readily available. This provides an unprecedented opportunity for knowledge discovery and data mining algorithms to dig insights from them, which can, later on, be helpful to the improvement of the quality of care delivery. Predictive modeling of clinical risk, including in-hospital mortality, hospital readmission, chronic disease onset, condition exacerbation, etc., from patient EHR, is one of the health data analytic problems that attract most of the interests. The reason …
The wide implementation of electronic health record (EHR) systems facilitates the collection of large-scale health data from real clinical settings. Despite the significant increase in adoption of EHR systems, this data remains largely unexplored, but presents a rich data source for knowledge discovery from patient health histories in tasks such as understanding disease correlations and predicting health outcomes. However, the heterogeneity, sparsity, noise, and bias in this data present many complex challenges. This complexity makes it difficult to translate potentially relevant information i…

Healthcare professionals have long envisioned using the enormous processing powers of computers to discover new facts and medical knowledge locked inside electronic health records. These vast medical archives contain time-resolved information about medical visits, tests and procedures, as well as outcomes, which together form individual patient journeys. By assessing the similarities among these journeys, it is possible to uncover clusters of common disease trajectories with shared health outcomes. The assignment of patient journeys to specific clusters may in turn serve as the basis for perso…

Auditing machine learning-based (ML) healthcare tools for bias is critical to preventing patient harm, especially in communities that disproportionately face health inequities. General frameworks are becoming increasingly available to measure ML fairness gaps between groups. However, ML for health (ML4H) auditing principles call for a contextual, patient-centered approach to model assessment. Therefore, ML auditing tools must be (1) better aligned with ML4H auditing principles and (2) able to illuminate and characterize communities vulnerable to the most harm. To address this gap, we propose s…

Electronic health records are an increasingly important resource for understanding the interactions between patient health, environment, and clinical decisions. In this paper we report an empirical study of predictive modeling of several patient outcomes using three state-of-the-art machine learning methods. Our primary goal is to validate the models by interpreting the importance of predictors in the final models. Central to interpretation is the use of feature importance scores, which vary depending on the underlying methodology. In order to assess feature importance, we compared univariate …

Due to the rapid growth of information available about individual patients, most physicians suffer from information overload when they review patient information in health information technology systems. In this manuscript, we present a novel hybrid dynamic and multi-collaborative filtering method to improve information retrieval from electronic health records. It also leverages the key idea of collaborative filtering, orig…
In this work, we modify and apply self-supervision techniques to the domain of medical health insurance claims. We model patients' healthcare claims history analogous to free-text narratives, and introduce pre-trained `prior knowledge', later utilized for patient outcome predictions on a challenging task: predicting Covid-19 hospitalization, given a patient's pre-Covid-19 insurance claims history. Results suggest that pre-training on insurance claims not only produces better prediction performance, but, more importantly, improves the model's `clinical trustworthiness' and model stability/relia…
The quality of healthcare provided by hospitals is subject to considerable variability. Consequently, accurate measurements of hospital performance are essential for various decision-makers, including patients, hospital managers and health insurers. Hospital performance is assessed via the health outcomes of their patients. However, as the risk profiles of patients between hospitals vary, measuring hospital performance requires adjustment for patient risk. This task is formalized in the state-of-the-art procedure through a hierarchical generalized linear model, that isolates hospital fixed-eff…

We applied method to experimental data in which the physical strain of subjects was systematically varied. We derived health states based on parameters extracted from ECG data. The occurrence of health states shows a high temporal correlation to the experimental phases of the physical exercise. We compared our…
