This paper deals with segmentation of organs at risk (OAR) in head and neck area in CT images which is a crucial step for reliable intensity modulated radiotherapy treatment. We introduce a convolution neural network with encoder-decoder architecture and a new loss function, the batch soft Dice loss function, used to train the network. The resulting model produces segmentations of every OAR in the public MICCAI 2015 Head And Neck Auto-Segmentation Challenge dataset. Despite the heavy class imbalance in the data, we improve accuracy of current state-of-the-art methods by 0.33 mm in terms of average surface distance and by 0.11 in terms of Dice overlap coefficient on average.

The segmentation of organs at risk (OAR) is a required precondition for the cancer treatment with image guided radiation therapy. The automation of the segmentation task is therefore of high clinical relevance. Deep Learning (DL) based medical image segmentation is currently the most successful approach, but suffers from the over-presence of the background class and the anatomically given organ size difference, which is most severe in the head and neck (HAN) area. To tackle the HAN area specific class imbalance problem we first optimize the patch-size of the currently best performing general p…

Deep learning-based methods achieved impressive results for the segmentation of medical images. With the development of 3D fully convolutional networks (FCNs), it has become feasible to produce improved results for multi-organ segmentation of 3D computed tomography (CT) images. The results of multi-organ segmentation using deep learning-based methods not only depend on the choice of networks architecture, but also strongly rely on the choice of loss function. In this paper, we present a discussion on the influence of Dice-based loss functions for multi-class organ segmentation using a dataset …

Radiotherapy is a treatment where radiation is used to eliminate cancer cells. The delineation of organs-at-risk (OARs) is a vital step in radiotherapy treatment planning to avoid damage to healthy organs. For nasopharyngeal cancer, more than 20 OARs are needed to be precisely segmented in advance. The challenge of this task lies in complex anatomical structure, low-contrast organ contours, and the extremely imbalanced size between large and small organs. Common segmentation methods that treat them equally would generally lead to inaccurate small-organ labeling. We propose a novel two-stage de…
In radiotherapy planning, manual contouring is labor-intensive and time-consuming. Accurate and robust automated segmentation models improve the efficiency and treatment outcome. We aim to develop a novel hybrid deep learning approach, combining convolutional neural networks (CNNs) and the self-attention mechanism, for rapid and accurate multi-organ segmentation on head and neck computed tomography (CT) images. Head and neck CT images with manual contours of 115 patients were retrospectively collected and used. We set the training/validation/testing ratio to 81/9/25 and used the 10-fold cross-…

Automatic segmentation of head and neck tumors plays an important role in radiomics analysis. In this short paper, we propose an automatic segmentation method for head and neck tumors from PET and CT images based on the combination of convolutional neural networks (CNNs) and hybrid active contours. Specifically, we first introduce a multi-channel 3D U-Net to segment the tumor with the concatenated PET and CT images. Then, we estimate the segmentation uncertainty by model ensembles and define a segmentation quality score to select the cases with high uncertainties. Finally, we develop a hybrid …

Accurate segmentation of organ at risk (OAR) play a critical role in the treatment planning of image guided radiation treatment of head and neck cancer. This segmentation task is challenging for both human and automatic algorithms because of the relatively large number of OARs to be segmented, the large variability of the size and morphology across different OARs, and the low contrast of between some OARs and the background. In this paper, we proposed a two-stage segmentation framework based on 3D U-Net. In this framework, the segmentation of each OAR is decomposed into two sub-tasks: locating…

In this paper, we propose an end-to-end deep neural network for solving the problem of imbalanced large and small organ segmentation in head and neck (HaN) CT images. To conduct radiotherapy planning for nasopharyngeal cancer, more than 10 organs-at-risk (normal organs) need to be precisely segmented in advance. However, the size ratio between large and small organs in the head could reach hundreds. Directly using such imbalanced organ annotations to train deep neural networks generally leads to inaccurate small-organ label maps. We propose a novel end-to-end deep neural network to solve this …

OAR segmentation is a critical step in radiotherapy of head and neck (H&N) cancer, where inconsistencies across radiation oncologists and prohibitive labor costs motivate automated approaches. However, leading methods using standard fully convolutional network workflows that are challenged when the number of OARs becomes large, e.g. > 40. For such scenarios, insights can be gained from the stratification approaches seen in manual clinical OAR delineation. This is the goal of our work, where we introduce stratified organ at risk segmentation (SOARS), an approach that stratifies OARs into anchor…

The accurate segmentation of organs-at-risk (OARs) in head and neck CT images is a critical step for radiation therapy of head and neck cancer patients. However, manual delineation for numerous OARs is time-consuming and laborious, even for expert oncologists. Moreover, manual delineation results are susceptible to high intra- and inter-variability. To this end, we propose a novel dual shape guided network (DSGnet) to automatically delineate nine important OARs in head and neck CT images. Radiation therapy (RT) is widely employed in the clinic for the treatment of head and neck (HaN) cancers. An essential step of RT planning is the accurate segmentation of various organs-at-risks (OARs) in HaN CT images. Nevertheless, segmenting OARs manually is time-consuming, tedious, and error-prone considering that typical HaN CT images contain tens to hundreds of slices. Automated segmentation algorithms are urgently required. Recently, convolutional neural networks (CNNs) have been extensively investigated on this task. Particularly, 3D CNNs are frequently adopted to process 3D HaN CT ima…

Accurate organ at risk (OAR) segmentation is critical to reduce the radiotherapy post-treatment complications. Consensus guidelines recommend a set of more than 40 OARs in the head and neck (H&N) region, however, due to the predictable prohibitive labor-cost of this task, most institutions choose a substantially simplified protocol by delineating a smaller subset of OARs and neglecting the dose distributions associated with other OARs.
