Nasopharyngeal Carcinoma (NPC) is a leading form of Head-and-Neck (HAN) cancer in the Arctic, China, Southeast Asia, and the Middle East/North Africa. Accurate segmentation of Organs-at-Risk (OAR) from Computed Tomography (CT) images with uncertainty information is critical for effective planning of radiation therapy for NPC treatment. Despite the stateof-the-art performance achieved by Convolutional Neural Networks (CNNs) for automatic segmentation of OARs, existing methods do not provide uncertainty estimation of the segmentation results for treatment planning, and their accuracy is still limited by several factors, including the low contrast of soft tissues in CT, highly imbalanced sizes of OARs and large inter-slice spacing. To address these problems, we propose a novel framework for accurate OAR segmentation with reliable uncertainty estimation. First, we propose a Segmental Linear Function (SLF) to transform the intensity of CT images to make multiple organs more distinguishable than existing methods based on a simple window width/level that often gives a better visibility of one organ while hiding the others. Second, to deal with the large inter-slice spacing, we introduce a novel 2.5D network (named as 3D-SepNet) specially designed for dealing with clinic HAN CT scans with anisotropic spacing. Thirdly, existing hardness-aware loss function often deal with class-level hardness, but our proposed attention to hard voxels (ATH) uses a voxel-level hardness strategy, which is more suitable to dealing with some hard regions despite that its corresponding class may be easy. Our code is now available at https://github.com/HiLab-git/SepNet.

Radiotherapy is a treatment where radiation is used to eliminate cancer cells. The delineation of organs-at-risk (OARs) is a vital step in radiotherapy treatment planning to avoid damage to healthy organs. For nasopharyngeal cancer, more than 20 OARs are needed to be precisely segmented in advance. The challenge of this task lies in complex anatomical structure, low-contrast organ contours, and the extremely imbalanced size between large and small organs. Common segmentation methods that treat them equally would generally lead to inaccurate small-organ labeling. We propose a novel two-stage de…
In radiotherapy planning, manual contouring is labor-intensive and time-consuming. Accurate and robust automated segmentation models improve the efficiency and treatment outcome. We aim to develop a novel hybrid deep learning approach, combining convolutional neural networks (CNNs) and the self-attention mechanism, for rapid and accurate multi-organ segmentation on head and neck computed tomography (CT) images. Head and neck CT images with manual contours of 115 patients were retrospectively collected and used. We set the training/validation/testing ratio to 81/9/25 and used the 10-fold cross-…

In this paper, we propose an end-to-end deep neural network for solving the problem of imbalanced large and small organ segmentation in head and neck (HaN) CT images. To conduct radiotherapy planning for nasopharyngeal cancer, more than 10 organs-at-risk (normal organs) need to be precisely segmented in advance. However, the size ratio between large and small organs in the head could reach hundreds. Directly using such imbalanced organ annotations to train deep neural networks generally leads to inaccurate small-organ label maps. We propose a novel end-to-end deep neural network to solve this …

Head and Neck (H\&N) organ-at-risk (OAR) and tumor segmentations are essential components of radiation therapy planning. The varying anatomic locations and dimensions of H\&N nodal Gross Tumor Volumes (GTVn) and H\&N primary gross tumor volume (GTVp) are difficult to obtain due to lack of accurate and reliable delineation methods. The downstream effect of incorrect segmentation can result in unnecessary irradiation of normal organs. Towards a fully automated radiation therapy planning algorithm, we explore the efficacy of multi-scale fusion based deep learning architectures for accurately segm…

The segmentation of organs at risk (OAR) is a required precondition for the cancer treatment with image guided radiation therapy. The automation of the segmentation task is therefore of high clinical relevance. Deep Learning (DL) based medical image segmentation is currently the most successful approach, but suffers from the over-presence of the background class and the anatomically given organ size difference, which is most severe in the head and neck (HAN) area. To tackle the HAN area specific class imbalance problem we first optimize the patch-size of the currently best performing general p…

The accurate segmentation of organs-at-risk (OARs) in head and neck CT images is a critical step for radiation therapy of head and neck cancer patients. However, manual delineation for numerous OARs is time-consuming and laborious, even for expert oncologists. Moreover, manual delineation results are susceptible to high intra- and inter-variability. To this end, we propose a novel dual shape guided network (DSGnet) to automatically delineate nine important OARs in head and neck CT images. To deal with the large shape variation and unclear boundary of OARs in CT images, we represent the organ s…

Radiation therapy (RT) is widely employed in the clinic for the treatment of head and neck (HaN) cancers. An essential step of RT planning is the accurate segmentation of various organs-at-risks (OARs) in HaN CT images. Nevertheless, segmenting OARs manually is time-consuming, tedious, and error-prone considering that typical HaN CT images contain tens to hundreds of slices. Automated segmentation algorithms are urgently required. Recently, convolutional neural networks (CNNs) have been extensively investigated on this task. Particularly, 3D CNNs are frequently adopted to process 3D HaN CT ima…

Deep learning models for organ contouring in radiotherapy are poised for clinical usage, but currently, there exist few tools for automated quality assessment (QA) of the predicted contours. Using Bayesian models and their associated uncertainty, one can potentially automate the process of detecting inaccurate predictions. We investigate two Bayesian models for auto-contouring, DropOut and FlipOut, using a quantitative measure - expected calibration error (ECE) and a qualitative measure - region-based accuracy-vs-uncertainty (R-AvU) graphs. It is well understood that a model should have low EC…

Purpose: Organ-at-risk (OAR) delineation is a key step for cone-beam CT (CBCT) based adaptive radiotherapy planning that can be a time-consuming, labor-intensive, and subject-to-variability process. We aim to develop a fully automated approach aided by synthetic MRI for rapid and accurate CBCT multi-organ contouring in head-and-neck (HN) cancer patients. MRI has superb soft-tissue contrasts, while CBCT offers bony-structure contrasts. Using the complementary information provided by MRI and CBCT is expected to enable accurate multi-organ segmentation in HN cancer patients. In our proposed metho…

Accurate organ at risk (OAR) segmentation is critical to reduce the radiotherapy post-treatment complications. Consensus guidelines recommend a set of more than 40 OARs in the head and neck (H&N) region, however, due to the predictable prohibitive labor-cost of this task, most institutions choose a substantially simplified protocol by delineating a smaller subset of OARs and neglecting the dose distributions associated with other OARs. In this work we propose a novel, automated and highly effective stratified OAR segmentation (SOARS) system using deep learning to precisely delineate a comprehe…
A 3D deep learning model (OARnet) is developed and used to delineate 28 H&N OARs on CT images. OARnet utilizes a densely connected network to detect the OAR bounding-box, then delineates the OAR within the box. It reuses information from any layer to subsequent layers and uses skip connections to combine information from different dense block levels to progressively improve delineation accuracy. Training uses up to 28 expert manual delineated (MD) OARs from 165 CTs. Dice similarity coefficient (DSC) and the 95th percentile Hausdorff distance (HD95) with respect to MD is assessed for 70 other C…

Nasopharyngeal carcinoma (NPC) is a kind of malignant tumor. Accurate and automatic segmentation of organs at risk (OAR) of computed tomography (CT) images is clinically significant. In recent years, deep learning models represented by U-Net have been widely applied in medical image segmentation tasks, which can help doctors with reduction of workload and get accurate results more quickly. In OAR segmentation of NPC, the sizes of OAR are variable, especially, some of them are small. Traditional deep neural networks underperform during segmentation due to the lack use of global and multi-size i…

Organs-at-risk (OAR) delineation in computed tomography (CT) is an important step in Radiation Therapy (RT) planning. Recently, deep learning based methods for OAR delineation have been proposed and applied in clinical practice for separate regions of the human body (head and neck, thorax, and abdomen). However, there are few researches regarding the end-to-end whole-body OARs delineation because the existing datasets are mostly partially or incompletely annotated for such task. In this paper, our proposed end-to-end convolutional neural network model, called \textbf{AttentionAnatomy}, can be …

Moreover, separable convolution and residual concatenation was introduced light UNet to reduce the computational cost. Further, a robust segmentation loss was applied to improve the performance. In the inference phase, only a light UNet is used, which required low time cost a…

Fully convolutional neural networks like U-Net have been the state-of-the-art methods in medical image segmentation. Practically, a network is highly specialized and trained separately for each segmentation task. Instead of a collection of multiple models, it is highly desirable to learn a universal data representation for different tasks, ideally a single model with the addition of a minimal number of parameters steered to each task. Inspired by the recent success of multi-domain learning in image classification, for the first time we explore a promising universal architecture that handles mu…
