As the global burden of Alzheimer's disease (AD) continues to grow, early and accurate detection has become increasingly critical, especially in regions with limited access to advanced diagnostic tools. We propose BRAINS (Biomedical Retrieval-Augmented Intelligence for Neurodegeneration Screening) to address this challenge. This novel system harnesses the powerful reasoning capabilities of Large Language Models (LLMs) for Alzheimer's detection and monitoring. BRAINS features a dual-module architecture: a cognitive diagnostic module and a case-retrieval module. The Diagnostic Module utilizes LLMs fine-tuned on cognitive and neuroimaging datasets -- including MMSE, CDR scores, and brain volume metrics -- to perform structured assessments of Alzheimer's risk. Meanwhile, the Case Retrieval Module encodes patient profiles into latent representations and retrieves similar cases from a curated knowledge base. These auxiliary cases are fused with the input profile via a Case Fusion Layer to enhance contextual understanding. The combined representation is then processed with clinical prompts for inference. Evaluations on real-world datasets demonstrate BRAINS effectiveness in classifying disease severity and identifying early signs of cognitive decline. This system not only shows strong potential as an assistive tool for scalable, explainable, and early-stage Alzheimer's disease detection, but also offers hope for future applications in the field.

Alzheimer's disease is a degenerative brain disease. Being the primary cause of Dementia in adults and progressively destroys brain memory. Though Alzheimer's disease does not have a cure currently, diagnosing it at an earlier stage will help reduce the severity of the disease. Thus, early diagnosis of Alzheimer's could help to reduce or stop the disease from progressing. In this paper, we proposed a deep convolutional neural network-based model for learning model using to determine the stage of Dementia in adults based on the Magnetic Resonance Imaging (MRI) images to detect the early onset o…

Brain network analysis is essential for diagnosing and intervention for Alzheimer's disease (AD). However, previous research relied primarily on specific time-consuming and subjective toolkits. Only few tools can obtain the structural brain networks from brain diffusion tensor images (DTI). In this paper, we propose a diffusion based end-to-end brain network generative model Brain Diffuser that directly shapes the structural brain networks from DTI. Compared to existing toolkits, Brain Diffuser exploits more structural connectivity features and disease-related information by analyzing disparit…
Alzheimer's Disease destroys brain cells causing people to lose their memory, mental functions and ability to continue daily activities. It is a severe neurological brain disorder which is not curable, but earlier detection of Alzheimer's Disease can help for proper treatment and to prevent brain tissue damage. Detection and classification of Alzheimer's Disease (AD) is challenging because sometimes the signs that distinguish Alzheimer's Disease MRI data can be found in normal healthy brain MRI data of older people. Moreover, there are relatively small amount of dataset available to train the …

Normative modeling is an emerging and promising approach to effectively study disorder heterogeneity in individual participants. In this study, we propose a novel normative modeling method by combining conditional variational autoencoder with adversarial learning (ACVAE) to identify brain dysfunction in Alzheimer's Disease (AD). Specifically, we first train a conditional VAE on the healthy control (HC) group to create a normative model conditioned on covariates like age, gender and intracranial volume. Then we incorporate an adversarial training process to construct a discriminative feature sp…

Characterizing the subtle changes of functional brain networks associated with the pathological cascade of Alzheimer's disease (AD) is important for early diagnosis and prediction of disease progression prior to clinical symptoms. We developed a new deep learning method, termed multiple graph Gaussian embedding model (MG2G), which can learn highly informative network features by mapping high-dimensional resting-state brain networks into a low-dimensional latent space. These latent distribution-based embeddings enable a quantitative characterization of subtle and heterogeneous brain connectivit…

Alzheimer's disease is characterized by alterations of the brain's structural and functional connectivity during its progressive degenerative processes. Existing auxiliary diagnostic methods have accomplished the classification task, but few of them can accurately evaluate the changing characteristics of brain connectivity. In this work, a prior guided adversarial representation learning and hypergraph perceptual network (PGARL-HPN) is proposed to predict abnormal brain connections using triple-modality medical images. Concretely, a prior distribution from the anatomical knowledge is estimated…
In recent days, Convolutional Neural Networks (CNN) have demonstrated impressive performance in medical image analysis. However, there is a lack of clear understanding of why and how the Convolutional Neural Network performs so well for image analysis task. How CNN analyzes an image and discriminates among samples of different classes are usually considered as non-transparent. As a result, it becomes difficult to apply CNN based approaches in clinical procedures and automated disease diagnosis systems. In this paper, we consider this issue and work on visualizing and understanding the decision…

Alzheimer's disease (AD) is associated with local (e.g. brain tissue atrophy) and global brain changes (loss of cerebral connectivity), which can be detected by high-resolution structural magnetic resonance imaging. Conventionally, these changes and their relation to AD are investigated independently. Here, we introduce a novel, highly-scalable approach that simultaneously captures $\textit{local}$ and $\textit{global}$ changes in the diseased brain. It is based on a neural network architecture that combines patch-based, high-resolution 3D-CNNs with global topological features, evaluating mult…

This work: (a) analyzes brain shape using surface information of the cortex and subcortical structures, (b) proposes a residual learning fr…
Neuroimaging biomarkers that distinguish between typical brain aging and Alzheimer's disease (AD) are valuable for determining how much each contributes to cognitive decline. However, substantial overlap between brain regions affected in the two processes confounds measuring them independently. We present a methodology toward disentangling the two. T1-we…

The framework starts with a data augmentation method called Brain-Aware Replacements (BAR), which leverages a standard brain parcellation to replace medically-relevant 3D brain regions in an anchor MRI from a randomly picked MRI to create synthetic samples. Ground truth "hard" labels are also linearly mixed depending on the replacement ratio in order to create "soft" labels. BAR produces a great variety of realistic-looking synthetic MRIs with higher local variability compared to other mix-based methods, such…

Accurate diagnosis of Alzheimer's disease (AD) is both challenging and time consuming. With a systematic approach for early detection and diagnosis of AD, steps can be taken towards the treatment and prevention of the disease. This study explores the practical application of deep learning models for diagnosis of AD. Due to computational complexity, large training times and limited availability of labelled dataset, a 3D full brain CNN (convolutional neural network) is not commonly used, and researchers often prefer 2D CNN variants. In this study, full brain 3D version of well-known 2D CNNs were…

Introspection of deep supervised predictive models trained on functional and structural brain imaging may uncover novel markers of Alzheimer's disease (AD). However, supervised training is prone to learning from spurious features (shortcut learning) impairing its value in the discovery process. Deep unsupervised and, recently, contrastive self-supervised approaches, not biased to classification, are better candidates for the task. Their multimodal options specifically offer additional regularization via modality interactions. In this paper, we introduce a way to exhaustively consider multimoda…
