The importance of radiomics features for predicting patient outcome is now well-established. Early study of prognostic features can lead to a more efficient treatment personalisation. For this reason new radiomics features obtained through mathematical morphology-based operations are proposed. Their study is conducted on an open database of patients suffering from Nonsmall Cells Lung Carcinoma (NSCLC). The tumor features are extracted from the CT images and analyzed via PCA and a Kaplan-Meier survival analysis in order to select the most relevant ones. Among the 1,589 studied features, 32 are found relevant to predict patient survival: 27 classical radiomics features and five MM features (including both granularity and morphological covariance features). These features will contribute towards the prognostic models, and eventually to clinical decision making and the course of treatment for patients.
Herein we propose a deep learning-based approach for the prediction of lung lesion response based on radiomic features extracted from clinical CT scans of patients in non-small cell lung cancer trials. The approach starts with the classification of lung lesions from the set of primary and metastatic lesions at various anatomic locations. Focusing on the lung lesions, we perform automatic segmentation to extract their 3D volumes. Radiomic features are then extracted from the lesion on the pre-treatment scan and the first follow-up scan to predict which lesions will shrink at least 30% in diamet…

As an analytic pipeline for quantitative imaging feature extraction and analysis, radiomics has grown rapidly in the past a few years. Recent studies in radiomics aim to investigate the relationship between tumors imaging features and clinical outcomes. Open source radiomics feature banks enable the extraction and analysis of thousands of predefined features. On the other hand, recent advances in deep learning have shown significant potential in the quantitative medical imaging field, raising the research question of whether predefined radiomics features have predictive information in addition…

Moreover, optimal number of features required for the classification task also needs to be evaluated. In this study, we investigate the impact of supervised and unsupervised feature selection techniques on machine learning methods for nodule classification in Computed Tomography (CT) images. The research work explores the classifica…

Spiculations/lobulations, sharp/curved spikes on the surface of lung nodules, are good predictors of lung cancer malignancy and hence, are routinely assessed and reported by radiologists as part of the standardized Lung-RADS clinical scoring criteria. Given the 3D geometry of the nodule and 2D slice-by-slice assessment by radiologists, manual spiculation/lobulation annotation is a tedious task and thus no public datasets exist to date for probing the importance of these clinically-reported features in the SOTA malignancy prediction algorithms. As part of this paper, we release a large-scale Cl…

Lesion segmentation is a crucial step of the radiomic workflow. Manual segmentation requires long execution time and is prone to variability, impairing the realisation of radiomic studies and their robustness. In this study, a deep-learning automatic segmentation method was applied on computed tomography images of non-small-cell lung cancer patients. The use of manual vs automatic segmentation in the performance of survival radiomic models was assessed, as well. METHODS A total of 899 NSCLC patients were included (2 proprietary: A and B, 1 public datasets: C). Automatic segmentation of lung le…
Identifying image features that are robust with respect to segmentation variability and domain shift is a tough challenge in radiomics. So far, this problem has mainly been tackled in test-retest analyses. In this work we analyze radiomics feature stability based on probabilistic segmentations. Based on a public lung cancer dataset, we generate an arbitrary number of plausible segmentations using a Probabilistic U-Net. From these segmentations, we extract a high number of plausible feature vectors for each lung tumor and analyze feature variance with respect to the segmentations. Our results s…

Early diagnosis of lung cancer is a key intervention for the treatment of lung cancer computer aided diagnosis (CAD) can play a crucial role. However, most published CAD methods treat lung cancer diagnosis as a lung nodule classification problem, which does not reflect clinical practice, where clinicians diagnose a patient based on a set of images of nodules, instead of one specific nodule. Besides, the low interpretability of the output provided by these methods presents an important barrier for their adoption. In this article, we treat lung cancer diagnosis as a multiple instance learning (M…

Pancreatic Ductal Adenocarcinoma (PDAC) is one of the most aggressive cancers with an extremely poor prognosis. Radiomics has shown prognostic ability in multiple types of cancer including PDAC. However, the prognostic value of traditional radiomics pipelines, which are based on hand-crafted radiomic features alone is limited. Convolutional neural networks (CNNs) have been shown to outperform these feature-based models in computer vision tasks. However, training a CNN from scratch needs a large sample size which is not feasible in most medical imaging studies. As an alternative solution, CNN-b…

Outcome prediction is crucial for head and neck cancer patients as it can provide prognostic information for early treatment planning. Radiomics methods have been widely used for outcome prediction from medical images. However, these methods are limited by their reliance on intractable manual segmentation of tumor regions. Recently, deep learning methods have been proposed to perform end-to-end outcome prediction so as to remove the reliance on manual segmentation. Unfortunately, without segmentation masks, these methods will take the whole image as input, such that makes them difficult to foc…

The feature extraction methods of radiomics are mainly based on static tomographic images at a certain moment, while the occurrence and development of disease is a dynamic process that cannot be fully reflected by only static characteristics. This study proposes a new dynamic radiomics feature extraction workflow that uses time-dependent tomographic images of the same patient, focuses on the changes in image features over time, and then quantifies them as new dynamic features for diagnostic or prognostic evaluation. We first define the mathematical paradigm of dynamic radiomics and introduce t…

Radiomics analysis has achieved great success in recent years. However, conventional Radiomics analysis suffers from insufficiently expressive hand-crafted features. Recently, emerging deep learning techniques, e.g., convolutional neural networks (CNNs), dominate recent research in Computer-Aided Diagnosis (CADx). Unfortunately, as black-box predictors, we argue that CNNs are "diagnosing" voxels (or pixels), rather than lesions; in other words, visual saliency from a trained CNN is not necessarily concentrated on the lesions. On the other hand, classification in clinical applications suffers f…

Non-small-cell lung cancer (NSCLC) represents approximately 80-85% of lung cancer diagnoses and is the leading cause of cancer-related death worldwide. Recent studies indicate that image-based radiomics features from positron emission tomography-computed tomography (PET/CT) images have predictive power on NSCLC outcomes. To this end, easily calculated functional features such as the maximum and the mean of standard uptake value (SUV) and total lesion glycolysis (TLG) are most commonly used for NSCLC prognostication, but their prognostic value remains controversial. Meanwhile, convolutional neu…

Cox proportional hazard model (CPH) is commonly used in clinical research for survival analysis. In quantitative medical imaging (radiomics) studies, CPH plays an important role in feature reduction and modeling. Survival prediction is a major concern for NPC patients, as it provides early prognostic information to plan treatments. Recently, deep survival models based on deep learning have demonstrated the potential to outperform traditional radiomics-based survival prediction models. Deep survival models usually use image patches covering the whole target regions (e.g., nasopharynx for NPC) or containing only segmented tumor regions as the input. The concept of tumor field effect implies that cancer is a systemic disease with its impact way beyond the visible tumor confines. For instance, in Glioblastoma (GBM), an aggressive brain tumor, the increase in intracranial pressure due to tumor burden often leads to brain herniation and poor outcomes. Our work is based on the rationale that highly aggressive tumors tend to grow uncontrollably, leading to pronounced biomechanical tissue deformations in the normal parenchyma, which when combined with local morphological differences in the tumor confines on MRI scans, will comprehensively captur…

Well-labeled datasets of chest radiographs (CXRs) are difficult to acquire due to the high cost of annotation. Thus, it is desirable to learn a robust and transferable representation in an unsupervised manner to benefit tasks that lack labeled data. Unlike natural images, medical images have their own domain prior; e.g., we observe that many pulmonary diseases, such as the COVID-19, manifest as changes in the lung tissue texture rather than the anatomical structure. Therefore, we hypothesize that studying only the texture without the influence of structure variations would be advantageous for …
