Early prediction of mortality and length of stay(LOS) of a patient is vital for saving a patient's life and management of hospital resources. Availability of electronic health records(EHR) makes a huge impact on the healthcare domain and there has seen several works on predicting clinical problems. However, many studies did not benefit from the clinical notes because of the sparse, and high dimensional nature. In this work, we extract medical entities from clinical notes and use them as additional features besides time-series features to improve our predictions. We propose a convolution based multimodal architecture, which not only learns effectively combining medical entities and time-series ICU signals of patients, but also allows us to compare the effect of different embedding techniques such as Word2vec, FastText on medical entities. In the experiments, our proposed method robustly outperforms all other baseline models including different multimodal architectures for all clinical tasks. The code for the proposed method is available at https://github.com/tanlab/ConvolutionMedicalNer.

Clinical concept extraction often begins with clinical Named Entity Recognition (NER). Often trained on annotated clinical notes, clinical NER models tend to struggle with tagging clinical entities in user queries because of the structural differences between clinical notes and user queries. User queries, unlike clinical notes, are often ungrammatical and incoherent. In many cases, user queries are compounded of multiple clinical entities, without comma or conjunction words separating them. By using as dataset a mixture of annotated clinical notes and synthesized user queries, we adapt a clini…

We hypothesize that explicit integration of contextual information into an Multi-task Learning framework would emphasize the significance of context for boosting performance in jointly learning Named Entity Recognition (NER) and Relation Extraction (RE). Our work proves this hypothesis by segmenting entities from their surrounding context and by building contextual representations using each independent segment. This relation representation allows for a joint NER/RE system that achieves near state-of-the-art (SOTA) performance on both NER and RE tasks while beating the SOTA RE system at end-to…

Owing to the exponential rise in the electronic medical records, information extraction in this domain is becoming an important area of research in recent years. Relation extraction between the medical concepts such as medical problem, treatment, and test etc. is also one of the most important tasks in this area. In this paper, we present an efficient relation extraction system based on the shortest dependency path (SDP) generated from the dependency parsed tree of the sentence. Instead of relying on many handcrafted features and the whole sequence of tokens present in a sentence, our system r…
Clinical text provides essential information to estimate the acuity of a patient during hospital stays in addition to structured clinical data. In this study, we explore how clinical text can complement a clinical predictive learning task. We leverage an internal medical natural language processing service to perform named entity extraction and negation detection on clinical notes and compose selected entities into a new text corpus to train document representations. We then propose a multimodal neural network to jointly train time series signals and unstructured clinical text representations …

Knowledge graph embedding methods learn embeddings of entities and relations in a low dimensional space which can be used for various downstream machine learning tasks such as link prediction and entity matching. Various graph convolutional network methods have been proposed which use different types of information to learn the features of entities and relations. However, these methods assign the same weight (importance) to the neighbors when aggregating the information, ignoring the role of different relations with the neighboring entities. To this end, we propose a relation-aware graph atten…

Automatically extracting useful information from electronic medical records along with conducting disease diagnoses is a promising task for both clinical decision support(CDS) and neural language processing(NLP). Most of the existing systems are based on artificially constructed knowledge bases, and then auxiliary diagnosis is done by rule matching. In this study, we present a clinical intelligent decision approach based on Convolutional Neural Networks(CNN), which can automatically extract high-level semantic information of electronic medical records and then perform automatic diagnosis witho…
The surging amount of biomedical literature & digital clinical records presents a growing need for text mining techniques that can not only identify but also semantically relate entities in unstructured data. In this paper we propose a text mining framework comprising of Named Entity Recognition (NER) and Relation Extraction (RE) models, which expands on previous work in three main ways. First, we introduce two new RE model architectures -- an accuracy-optimized one based on BioBERT and a speed-optimized one utilizing crafted features over a Fully Connected Neural Network (FCNN). Second, we ev…

Clinical performance audits are routinely performed in Emergency Medical Services (EMS) to ensure adherence to treatment protocols, to identify individual areas of weakness for remediation, and to discover systemic deficiencies to guide the development of the training syllabus. At present, these audits are performed by manual chart review which is time-consuming and laborious. In this paper, we present an automatic audit system based on both the structured and unstructured ambulance case records and clinical notes with a deep neural network-based named entities recognition model. The patient with ischemic stroke can benefit most from the earliest possible definitive diagnosis. While the high quality medical resources are quite scarce across the globe, an automated diagnostic tool is expected in analyzing the magnetic resonance (MR) images to provide reference in clinical diagnosis. In this paper, we propose a deep learning method to automatically segment ischemic stroke lesions from multi-modal MR images. By using atrous convolution and global convolution network, our proposed residual-structured fully convolutional network (Res-FCN) is able to capture features from la…
Epileptic seizure detection and classification in clinical electroencephalogram data still is a challenge, and only low sensitivity with a high rate of false positives has been achieved with commercially available seizure detection tools, which usually are patient non-specific. Epilepsy patients suffer from severe detrimental effects like physical injury or depression due to unpredictable seizures. However, even in hospitals due to the high rate of false positives, the seizure alert systems are of poor help for patients as tools of seizure detection are mostly trained on unrealistically clean …

Medical health records and clinical summaries contain a vast amount of important information in textual form that can help advancing research on treatments, drugs and public health. However, the majority of these information is not shared because they contain private information about patients, their families, or medical staff treating them. Regulations such as HIPPA in the US, PHIPPA in Canada and GDPR regulate the protection, processing and distribution of this information. In case this information is de-identified and personal information are replaced or redacted, they could be distributed …

Automated medical coding is a process of codifying clinical notes to appropriate diagnosis and procedure codes automatically from the standard taxonomies such as ICD (International Classification of Diseases) and CPT (Current Procedure Terminology). The manual coding process involves the identification of entities from the clinical notes followed by querying a commercial or non-commercial medical codes Information Retrieval (IR) system that follows the Centre for Medicare and Medicaid Services (CMS) guidelines. We propose to automate this manual process by automatically constructing a query fo…
In the electronic health record, using clinical notes to identify entities such as disorders and their temporality (e.g. the order of an event relative to a time index) can inform many important analyses. However, creating training data for clinical entity tasks is time consuming and sharing labeled data is challenging due to privacy concerns. The information needs of the COVID-19 pandemic highlight the need for agile methods of training machine learning models for clinical notes. We present Trove, a framework for weakly supervised entity classification using medical ontologies and expert-gene…
Method: We develop CNN-based methods for automatic ICD coding based on clinical text from intensive care unit (ICU) stays. We come up with the Shallow and Wide Attention convolutional Mechanism (SWAM), which allows our model to learn local and low-level features for each label. The key idea behind our model design is to look for the presence of informative snippets in the clinical text that correlated with each code, and we infer that there exists a correspondence between "informative snippet" and convolution filter. Results: We evaluate our approach on MIMIC-III, an open-access dataset of ICU…

Skin lesions are conditions that appear on a patient due to many different reasons. One of these can be because of an abnormal growth in skin tissue, defined as cancer. This disease plagues more than 14.1 million patients and had been the cause of more than 8.2 million deaths, worldwide. Therefore, the construction of a classification model for 12 lesions, including Malignant Melanoma and Basal Cell Carcinoma, is proposed. Furthermore, in this work, it is used a ResNet-152 architecture, which was trained over 3,797 images, later augmented by a factor of 29 times, using positional, scale, and l…

We consider the problem of modeling the dynamics of continuous spatial-temporal processes represented by irregular samples through both space and time. Such processes occur in sensor networks, citizen science, multi-robot systems, and many others. We propose a new deep model that is able to directly learn and predict over this irregularly sampled data, without voxelization, by leveraging a recent convolutional architecture for static point clouds. The model also easily incorporates the notion of multiple entities in the process. In particular, the model can flexibly answer prediction queries a…
Clinical notes are text documents that are created by clinicians for each patient encounter. They are typically accompanied by medical codes, which describe the diagnosis and treatment. Annotating these codes is labor intensive and error prone; furthermore, the connection between the codes and the text is not annotated, obscuring the reasons and details behind specific diagnoses and treatments. We present an attentional convolutional network that predicts medical codes from clinical text. Our method aggregates information across the document using a convolutional neural network, and uses an at…
Feature engineering remains a major bottleneck when creating predictive systems from electronic medical records. We present Deepr (short for Deep record), a new end-to-end deep learning system that learns to extract features from medical records and predicts future risk automatically. Deepr transforms a record into a sequence of discrete elements separated by coded time gaps and hospital transfers. On top of the sequence is a convolutional neural net that detects and combin…

These GCNs capabilities have spawned a new wave of research in medical imaging analysis with the overarching goal of improving quantitative disease understanding, mon…
Semantic textual similarity (STS) in the clinical domain helps improve diagnostic efficiency and produce concise texts for downstream data mining tasks. However, given the high degree of domain knowledge involved in clinic text, it remains challenging for general language models to infer implicit medical relationships behind clinical sentences and output similarities correctly. Interpretability in Graph Convolutional Networks (GCNs) has been explored to some extent in computer vision in general, yet, in the medical domain, it requires further examination. Moreover, most of the interpretability approaches for GCNs, especially in the medical domain, focus on interpreting the model in a post hoc fashion. In this paper, we propose an interpretable graph learning-based model which 1) interprets the clinical relevance of the input features towards the task, 2) uses the explanation to improve the model performance and, 3) learns a population level latent graph that may be u…
Predicting the health risks of patients using Electronic Health Records (EHR) has attracted considerable attention in recent years, especially with the development of deep learning techniques. Health risk refers to the probability of the occurrence of a specific health outcome for a specific patient. The predicted risks can be used to support decision-making by healthcare professionals. EHRs are structured patient journey data. Each patient journey contains a chronological set of clinical events, and within each clinical event, there is a set of clinical/medical activities. Due to variations o…
Encoding facts as representations of entities and binary relationships between them, as learned by knowledge graph representation models, is useful for various tasks, including predicting new facts, question answering, fact checking and information retrieval. The focus of this thesis is on (i) improving knowledge graph representation with the aim of tackling the link prediction task; and (ii) devising a theory on how semantics can be captured in the geometry of relation representations. Most knowledge graphs are very incomplete and manually adding new information is costly, which drives the de…
