In this paper we present a method for simultaneously segmenting brain tumors and an extensive set of organs-at-risk for radiation therapy planning of glioblastomas. The method combines a contrast-adaptive generative model for whole-brain segmentation with a new spatial regularization model of tumor shape using convolutional restricted Boltzmann machines. We demonstrate experimentally that the method is able to adapt to image acquisitions that differ substantially from any available training data, ensuring its applicability across treatment sites; that its tumor segmentation accuracy is comparable to that of the current state of the art; and that it captures most organs-at-risk sufficiently well for radiation therapy planning purposes. The proposed method may be a valuable step towards automating the delineation of brain tumors and organs-at-risk in glioblastoma patients undergoing radiation therapy.

Automatic localization and segmentation of organs-at-risk (OAR) in CT are essential pre-processing steps in medical image analysis tasks, such as radiation therapy planning. For instance, the segmentation of OAR surrounding tumors enables the maximization of radiation to the tumor area without compromising the healthy tissues. However, the current medical workflow requires manual delineation of OAR, which is prone to errors and is annotator-dependent. In this work, we aim to introduce a unified 3D pipeline for OAR localization-segmentation rather than novel localization or segmentation archite…

Head and Neck (H\&N) organ-at-risk (OAR) and tumor segmentations are essential components of radiation therapy planning. The varying anatomic locations and dimensions of H\&N nodal Gross Tumor Volumes (GTVn) and H\&N primary gross tumor volume (GTVp) are difficult to obtain due to lack of accurate and reliable delineation methods. The downstream effect of incorrect segmentation can result in unnecessary irradiation of normal organs. Towards a fully automated radiation therapy planning algorithm, we explore the efficacy of multi-scale fusion based deep learning architectures for accurately segm…

Synthesized medical images have several important applications, e.g., as an intermedium in cross-modality image registration and as supplementary training samples to boost the generalization capability of a classifier. Especially, synthesized computed tomography (CT) data can provide X-ray attenuation map for radiation therapy planning. In this work, we propose a generic cross-modality synthesis approach with the following targets: 1) synthesizing realistic looking 3D images using unpaired training data, 2) ensuring consistent anatomical structures, which could be changed by geometric distorti…

Medical image analysis, especially segmenting a specific organ, has an important role in developing clinical decision support systems. In cardiac magnetic resonance (MR) imaging, segmenting the left and right ventricles helps physicians diagnose different heart abnormalities. There are challenges for this task, including the intensity and shape similarity between left ventricle and other organs, inaccurate boundaries and presence of noise in most of the images. In this paper we propose an automated method for segmenting the left ventricle in cardiac MR images. We first automatically extract th…

With the advancement of treatment modalities in radiation therapy for cancer patients, outcomes have improved, but at the cost of increased treatment plan complexity and planning time. The accurate prediction of dose distributions would alleviate this issue by guiding clinical plan optimization to save time and maintain high quality plans. We have modified a convolutional deep network model, U-net (originally designed for segmentation purposes), for predicting dose from patient image contours of the planning target volume (PTV) and organs at risk (OAR). We show that, as an example, we are able…
Due to the large combinatorial problem, current beam orientation optimization algorithms for radiotherapy, such as column generation (CG), are typically heuristic or greedy in nature, leading to suboptimal solutions. We propose a reinforcement learning strategy using Monte Carlo Tree Search capable of finding a superior beam orientation set and in less time than CG.We utilized a reinforcement learning structure involving a supervised learning network to guide Monte Carlo tree search (GTS) to explore the decision space of beam orientation selection problem. We have previously trained a deep neu…

Accurate segmenting nuclei instances is a crucial step in computer-aided image analysis to extract rich features for cellular estimation and following diagnosis as well as treatment. While it still remains challenging because the wide existence of nuclei clusters, along with the large morphological variances among different organs make nuclei instance segmentation susceptible to over-/under-segmentation. Additionally, the inevitably subjective annotating and mislabeling prevent the network learning from reliable samples and eventually reduce the generalization capability for robustly segmentin…

The liver is one of the most critical metabolic organs in vertebrates due to its vital functions in the human body, such as detoxification of the blood from waste products and medications. Liver diseases due to liver tumors are one of the most common mortality reasons around the globe. Hence, detecting liver tumors in the early stages of tumor development is highly required as a critical part of medical treatment. Many imaging modalities can be used as aiding tools to detect liver tumors. Computed tomography (CT) is the most used imaging modality for soft tissue organs such as the liver. This …
Radiation therapy has emerged as one of the preferred techniques to treat brain cancer patients. During treatment, a very high dose of radiation is delivered to a very narrow area. Prescribed radiation therapy for brain cancer requires precisely defining the target treatment area, as well as delineating vital brain structures which must be spared from radiotoxicity. Nevertheless, delineation task is usually still manually performed, which is inefficient and operator-dependent. Several attempts of automatizing this process have reported. however, marginal results when analyzing organs in the op…
Automated segmentation can assist radiotherapy treatment planning by saving manual contouring efforts and reducing intra-observer and inter-observer variations. The recent development of deep learning approaches has revoluted medical data processing, including semantic segmentation, by dramatically improving performance. However, training effective deep learning models usually require a large amount of high-quality labeled data, which are often costly to collect. We developed a novel semi-supervised adversarial deep learning approach for 3D pelvic CT image semantic segmentation. Unlike supervi…

Organs-at-risk (OAR) delineation in computed tomography (CT) is an important step in Radiation Therapy (RT) planning. Recently, deep learning based methods for OAR delineation have been proposed and applied in clinical practice for separate regions of the human body (head and neck, thorax, and abdomen). However, there are few researches regarding the end-to-end whole-body OARs delineation because the existing datasets are mostly partially or incompletely annotated for such task. In this paper, our proposed end-to-end convolutional neural network model, called \textbf{AttentionAnatomy}, can be …

Accurate segmentation of organs-at-risks (OARs) is a precursor for optimizing radiation therapy planning. Existing deep learning-based multi-scale fusion architectures have demonstrated a tremendous capacity for 2D medical image segmentation. The key to their success is aggregating global context and maintaining high resolution representations. However, when translated into 3D segmentation problems, existing multi-scale fusion architectures might underperform due to their heavy computation overhead and substantial data diet. To address this issue, we propose a new OAR segmentation framework, c…

The accurate segmentation of organs-at-risk (OARs) in head and neck CT images is a critical step for radiation therapy of head and neck cancer patients. However, manual delineation for numerous OARs is time-consuming and laborious, even for expert oncologists. Moreover, manual delineation results are susceptible to high intra- and inter-variability. To this end, we propose a novel dual shape guided network (DSGnet) to automatically delineate nine important OARs in head and neck CT images. To deal with the large shape variation and unclear boundary of OARs in CT images, we represent the organ s…

Planning of radiotherapy involves accurate segmentation of a large number of organs at risk, i.e. organs for which irradiation doses should be minimized to avoid important side effects of the therapy. We propose a deep learning method for segmentation of organs at risk inside the brain region, from Magnetic Resonance (MR) images. Our system performs segmentation of eight structures: eye, lens, optic nerve, optic chiasm, pituitary gland, hippocampus, brainstem and brain. We propose an efficient algorithm to train neural networks for an end-to-end segmentation of multiple and non-exclusive class…

Nasopharyngeal Carcinoma (NPC) is a leading form of Head-and-Neck (HAN) cancer in the Arctic, China, Southeast Asia, and the Middle East/North Africa. Accurate segmentation of Organs-at-Risk (OAR) from Computed Tomography (CT) images with uncertainty information is critical for effective planning of radiation therapy for NPC treatment. Despite the stateof-the-art performance achieved by Convolutional Neural Networks (CNNs) for automatic segmentation of OARs, existing methods do not provide uncertainty estimation of the segmentation results for treatment planning, and their accuracy is still li…

In current clinical practice, noisy and artifact-ridden weekly cone-beam computed tomography (CBCT) images are only used for patient setup during radiotherapy. Treatment planning is done once at the beginning of the treatment using high-quality planning CT (pCT) images and manual contours for organs-at-risk (OARs) structures. If the quality of the weekly CBCT images can be improved while simultaneously segmenting OAR structures, this can provide critical information for adapting radiotherapy mid-treatment as well as for deriving biomarkers for treatment response. Using a novel physics-based da…
