Automatic medical image segmentation is a crucial topic in the medical domain and successively a critical counterpart in the computer-aided diagnosis paradigm. U-Net is the most widespread image segmentation architecture due to its flexibility, optimized modular design, and success in all medical image modalities. Over the years, the U-Net model achieved tremendous attention from academic and industrial researchers. Several extensions of this network have been proposed to address the scale and complexity created by medical tasks. Addressing the deficiency of the naive U-Net model is the foremost step for vendors to utilize the proper U-Net variant model for their business. Having a compendium of different variants in one place makes it easier for builders to identify the relevant research. Also, for ML researchers it will help them understand the challenges of the biological tasks that challenge the model. To address this, we discuss the practical aspects of the U-Net model and suggest a taxonomy to categorize each network variant. Moreover, to measure the performance of these strategies in a clinical application, we propose fair evaluations of some unique and famous designs on well-known datasets. We provide a comprehensive implementation library with trained models for future research. In addition, for ease of future studies, we created an online list of U-Net papers with their possible official implementation. All information is gathered in https://github.com/NITR098/Awesome-U-Net repository.

Transformers have achieved remarkable success in medical image analysis owing to their powerful capability to use flexible self-attention mechanism. However, due to lacking intrinsic inductive bias in modeling visual structural information, they generally require a large-scale pre-training schedule, limiting the clinical applications over expensive small-scale medical data. To this end, we propose a parameter-efficient transformer to explore intrinsic inductive bias via position information for medical image segmentation. Specifically, we empirically investigate how different position encoding…

Convolutional Neural Networks (CNNs) have been successful in solving tasks in computer vision including medical image segmentation due to their ability to automatically extract features from unstructured data. However, CNNs are sensitive to rotation and affine transformation and their success relies on huge-scale labeled datasets capturing various input variations. This network paradigm has posed challenges at scale because acquiring annotated data for medical segmentation is expensive, and strict privacy regulations. Furthermore, visual representation learning with CNNs has its own flaws, e.g…

Semantic image segmentation is the process of labeling each pixel of an image with its corresponding class. An encoder-decoder based approach, like U-Net and its variants, is a popular strategy for solving medical image segmentation tasks. To improve the performance of U-Net on various segmentation tasks, we propose a novel architecture called DoubleU-Net, which is a combination of two U-Net architectures stacked on top of each other. The first U-Net uses a pre-trained VGG-19 as the encoder, which has already learned features from ImageNet and can be transferred to another task easily. To capt…

Medical image segmentation can provide a reliable basis for further clinical analysis and disease diagnosis. The performance of medical image segmentation has been significantly advanced with the convolutional neural networks (CNNs). However, most existing CNNs-based methods often produce unsatisfactory segmentation mask without accurate object boundaries. This is caused by the limited context information and inadequate discriminative feature maps after consecutive pooling and convolution operations. In that the medical image is characterized by the high intra-class variation, inter-class indi…

Fully Convolutional Neural Networks (FCNNs) with contracting and expanding paths have shown prominence for the majority of medical image segmentation applications since the past decade. In FCNNs, the encoder plays an integral role by learning both global and local features and contextual representations which can be utilized for semantic output prediction by the decoder. Despite their success, the locality of convolutional layers in FCNNs, limits the capability of learning long-range spatial dependencies. Recent advances of semantic image segmentation greatly benefit from deeper and larger Convolutional Neural Network (CNN) models. Compared to image segmentation in the wild, properties of both medical images themselves and of existing medical datasets hinder training deeper and larger models because of overfitting. We demonstrate that two-stream CNNs with more low-level features gre…
The shapes and morphology of the organs and tissues are important prior knowledge in medical imaging recognition and segmentation. The morphological operation is a well-known method for morphological feature extraction. As the morphological operation is performed well in hand-crafted image segmentation techniques, it is also promising to design an approach to approximate morphological operation in the convolutional networks. However, using the traditional convolutional neural network as a black-box is usually hard to specify the morphological operation action. Here, we introduced a 3D morpholo…

This paper proposes a novel U-Net variant using stacked dilated convolutions for medical image segmentation (SDU-Net). SDU-Net adopts the architecture of vanilla U-Net with modifications in the encoder and decoder operations (an operation indicates all the processing for feature maps of the same resolution). Unlike vanilla U-Net which incorporates two standard convolutions in each encoder/decoder operation, SDU-Net uses one standard convolution followed by multiple dilated convolutions and concatenates all dilated convolution outputs as input to the next operation. Experiments showed that SDU-…

We propose an attention mechanism for 3D medical image segmentation. The method, named segmentation-by-detection, is a cascade of a detection module followed by a segmentation module. The detection module enables a region of interest to come to attention and produces a set of object region candidates which are further used as an attention model. Rather than dealing with the entire volume, the segmentation module distills the information from the potential region. This scheme is an efficient solution for volumetric data as it reduces the influence of the surrounding noise which is especially im…

Although medical imaging data repositories continue to expand, there has not been a commensurate increase in the amount of annotated data. Hence, we propose a new spatial guided self-supervised clustering network (SGSCN) for medical image segmentation, where we introduce multiple loss functions designed to aid in gro…

Despite the tremendous success of deep neural networks in medical image segmentation, they typically require a large amount of costly, expert-level annotated data. Few-shot segmentation approaches address this issue by learning to transfer knowledge from limited quantities of labeled examples. Incorporating appropriate prior knowledge is critical in designing high-performance few-shot segmentation algorithms. Since strong spatial priors exist in many medical imaging modalities, we propose a prototype-based method -- namely, the location-sensitive local prototype network -- that leverages spati…
We develop and approach to unsupervised semantic medical image segmentation that extends previous work with generative adversarial networks. We use existing edge detection methods to construct simple edge diagrams, train a generative model to convert them into synthetic medical images, and construct a dataset of synthetic images with known segmentations using variations on extracted edge diagrams. This synthetic dataset is then used to train a supervised image segmentation model. We test our approach on a clinical dataset of kidney ultrasound images and the benchmark ISIC 2018 skin lesion data…
The task of localizing and categorizing objects in medical images often remains formulated as a semantic segmentation problem. This approach, however, only indirectly solves the coarse localization task by predicting pixel-level scores, requiring ad-hoc heuristics when mapping back to object-level scores. State-of-the-art object detectors on the other hand, allow for individual object scoring in an end-to-end fashion, while ironically trading in the ability to exploit the full pixel-wise supervision signal. This can be particularly disadvantageous in the setting of medical image analysis, wher…
Recently, neural architecture search (NAS) has been applied to automatically search high-performance networks for medical image segmentation. The NAS search space usually contains a network topology level (controlling connections among cells with different spatial scales) and a cell level (operations within each cell). Existing methods either require long searching time for large-scale 3D image datasets, or are limited to pre-defined topologies (such as U-shaped or single-path). In this work, we focus on three important aspects of NAS in 3D medical image segmentation: flexible multi-path netwo…

The U-Net was presented in 2015. With its straight-forward and successful architecture it quickly evolved to a commonly used benchmark in medical image segmentation. The adaptation of the U-Net to novel problems, however, comprises several degrees of freedom regarding the exact architecture, preprocessing, training and inference. These choices are not independent of each other and substantially impact the overall performance. The present paper introduces the nnU-Net ('no-new-Net'), which refers to a robust and self-adapting framework on the basis of 2D and 3D vanilla U-Nets. We argue the stron…

Coarse-to-fine models and cascade segmentation architectures are widely adopted to solve the problem of large scale variations in medical image segmentation. However, those methods have two primary limitations: the first-stage segmentation becomes a performance bottleneck; the lack of overall differentiability makes the training process of two stages asynchronous and inconsistent. In this paper, we propose a differentiable two-stage network architecture to tackle these problems. In the first stage, a localization network (L-Net) locates Regions of Interest (RoIs) in a detection fashion; in the…

Deep neural networks have been a prevailing technique in the field of medical image processing. However, the most popular convolutional neural networks (CNNs) based methods for medical image segmentation are imperfect because they model long-range dependencies by stacking layers or enlarging filters.
