Knowledge of x-ray attenuation is essential for developing and evaluating x-ray imaging technologies. For instance, techniques to distinguish between cysts and solid tumours at mammography screening would be highly desirable to reduce recalls, but the development requires knowledge of the x-ray attenuation for cysts and tumours. We have previously measured the attenuation of cyst fluid using photon-counting spectral mammography. Data on x-ray attenuation for solid breast lesions are available in the literature, but cover a relatively wide range, likely caused by natural spread between samples, random measurement errors, and different experimental conditions. In this study, we have adapted the previously developed spectral method to measure the linear attenuation of solid breast lesions. A total of 56 malignant and 5 benign lesions were included in the study. The samples were placed in a holder that allowed for thickness measurement. Spectral (energy-resolved) images of the samples were acquired and the image signal was mapped to equivalent thicknesses of two known reference materials, which can be used to derive the x-ray attenuation as a function of energy. The spread in equivalent material thicknesses was relatively large between samples, which is likely to be caused mainly by natural variation and only to a minor extent by random measurement errors and sample inhomogeneity. No significant difference in attenuation was found between benign and malignant solid lesions, or between different types of malignant lesions. The separation between cyst-fluid and tumour attenuation was, however, significant, which suggests it may be possible to distinguish cystic from solid breast lesions, and the results lay the groundwork for a clinical trial. A system for the automated detection of massive lesions in mammograms is presented. The approach we adopted is a pixel-based and multi-level one. Each pixel in a mammogram is flagged with the appropriate class membership, e.g. massive lesions or normal breast tissue.
Knowledge of x-ray attenuation is essential for developing and evaluating x-ray imaging technologies. For instance, techniques to better characterize cysts at mammography screening would be highly desirable to reduce recalls, but the development is hampered by the lack of attenuation data for cysts. We have developed a method to measure x-ray attenuation of tissue samples using a prototype photon-counting spectral mammography unit. The method was applied to measure the attenuation of 50 samples of breast cyst fluid and 50 samples of water. Spectral (energy-resolved) images of the samples were …
In this paper we introduce a computational modelling that reproduces the breast compression processes used to obtain the mammogram. The main result is a programme in which one can track the first steps of virtual mammography. On the one hand, our modelling enables addition of structures that represent different tissues, muscles and glands in the breast. On the other hand, we shall validate and implement it by means of laboratory tests with phantoms. To the best of our knowledge, these two characteristics do confer originality to our research. This is because their interrelation seems not to be…
We present a novel method for characterizing mammographic findings using spectral imaging without the use of contrast agent. Within a statistical framework, suspicious findings are analyzed to determine if they are likely to be benign cystic lesions or malignant tissue. To evaluate the method, we have designed a phantom where combinations of different tissue types are realized by decomposition into the material bases aluminum and polyethylene. The results indicate that the lesion size limit for reliable characterization is below 10 mm diameter, when quantum noise is the only considered source …
Background \& purpose: The recent emergence of neural networks models for the analysis of breast images has been a breakthrough in computer aided diagnostic. This approach was not yet developed in Contrast Enhanced Spectral Mammography (CESM) where access to large databases is complex. This work proposes a deep-learning-based Computer Aided Diagnostic development for CESM recombined images able to detect lesions and classify cases. Material \& methods: A large CESM diagnostic dataset with biopsy-proven lesions was collected from various hospitals and different acquisition systems. The annotate…
Positron Emission Mammography imaging system (PEMi) is a nuclear medicine diagnosis method dedicated for breast imaging. It provides a better resolution in detection of millimeter-sized breast tumors than whole-body PET. To address the requirement of semi-quantitative analysis with the radiotracer concentration map of the breast, a new attenuation correction method based on three-dimensional seeded region growing image segmentation (3DSRG-AC) solution was developed. The method gives a 3D connected region as the segmentation result instead of image slices. The continuously segmentation property…

The improved diagnostic accuracy of ultrasound breast examinations remains an important goal. In this study, we propose a biophysical feature based machine learning method for breast cancer detection to improve the performance beyond a benchmark deep learning algorithm and to furthermore provide a color overlay visual map of the probability of malignancy within a lesion. This overall framework is termed disease specific imaging. Previously, 150 breast lesions were segmented and classified utilizing a modified fully convolutional network and a modified GoogLeNet, respectively. In this study mul…
Mammography is currently the primary imaging modality for breast cancer screening and plays an important role in cancer diagnostics. A standard mammographic image acquisition always includes the compression of the breast prior x-ray exposure. The breast is compressed between two plates (the image receptor and the compression paddle) until a nearly uniform breast thickness is obtained. The breast flattening improves diagnostic image quality 1 and reduces the absorbed dose 2. However, this technique can also be a source of discomfort and might deter some women from attending breast screening by …

Purpose: Spectral imaging is a method in medical x-ray imaging to extract information about the object constituents by the material-specific energy dependence of x-ray attenuation. The authors have investigated a photon-counting spectral imaging system with two energy bins for contrast-enhanced mammography. System optimization and the potential benefit compared to conventional non-energy-resolved absorption imaging was studied. Methods: A framework for system characterization was set up that included quantum and anatomical noise and a theoretical model of the system was benchmarked to phantom …
It has previously been shown that 2D spectral mammography can be used to discriminate between (likely benign) cystic and (potentially malignant) solid lesions in order to reduce unnecessary recalls in mammography. One limitation of the technique is, however, that the composition of overlapping tissue needs to be interpolated from a region surrounding the lesion. The purpose of this investigation was to demonstrate that lesion characterization can be done with spectral tomosynthesis, and to investigate whether the 3D information available in tomosynthesis can reduce the uncertainty from the int…
Breast ultrasound (US) is an effective imaging modality for breast cancer detection and diagnosis. US computer-aided diagnosis (CAD) systems have been developed for decades and have employed either conventional hand-crafted features or modern automatic deep-learned features, the former relying on clinical experience and the latter demanding large datasets. In this paper, we have developed a novel BIRADS-SDL network that integrates clinically-approved breast lesion characteristics (BIRADS features) into semi-supervised deep learning (SDL) to achieve accurate diagnoses with a small training data…

Deep learning algorithms, especially convolutional neural networks, have become a methodology of choice in medical image analysis. However, recent studies in computer vision show that even a small modification of input image intensities may cause a deep learning model to classify the image differently. In medical imaging, the distribution of image intensities is related to applied image reconstruction algorithm. In this paper we investigate the impact of ultrasound image reconstruction method on breast lesion classification with neural transfer learning. Due to high dynamic range raw ultrasoni…
Photoacoustic imaging has proven to be able to detect vascularization-driven optical absorption contrast associated with tumors. In order to detect breast tumors located a few centimeter deep in tissue, a sensitive ultrasound detector is of crucial importance for photoacoustic mammography. Further, because the expected photoacoustic frequency bandwidth (a few MHz to tens of kHz) is inversely proportional to the dimensions of light absorbing structures (0.5 to 10+ mm), proper choices of materials and their geometries, and proper considerations in design have to be made for optimal photoacoustic…
