Recognition of surgical gesture is crucial for surgical skill assessment and efficient surgery training. Prior works on this task are based on either variant graphical models such as HMMs and CRFs, or deep learning models such as Recurrent Neural Networks and Temporal Convolutional Networks. Most of the current approaches usually suffer from over-segmentation and therefore low segment-level edit scores. In contrast, we present an essentially different methodology by modeling the task as a sequential decision-making process. An intelligent agent is trained using reinforcement learning with hierarchical features from a deep model. Temporal consistency is integrated into our action design and reward mechanism to reduce over-segmentation errors. Experiments on JIGSAWS dataset demonstrate that the proposed method performs better than state-of-the-art methods in terms of the edit score and on par in frame-wise accuracy. Our code will be released later.

Surgical gesture recognition is important for surgical data science and computer-aided intervention. Even with robotic kinematic information, automatically segmenting surgical steps presents numerous challenges because surgical demonstrations are characterized by high variability in style, duration and order of actions. In order to extract discriminative features from the kinematic signals and boost recognition accuracy, we propose a multi-task recurrent neural network for simultaneous recognition of surgical gestures and estimation of a novel formulation of surgical task progress. To show the…

Modeling and recognition of surgical activities poses an interesting research problem. Although a number of recent works studied automatic recognition of surgical activities, generalizability of these works across different tasks and different datasets remains a challenge. We introduce a modality that is robust to scene variation, and that is able to infer part information such as orientational and relative spatial relationships. The proposed modality is based on spatial temporal graph representations of surgical tools in videos, for surgical activity recognition. To explore its effectiveness,…

Data diversity and volume are crucial to the success of training deep learning models, while in the medical imaging field, the difficulty and cost of data collection and annotation are especially huge. Specifically in robotic surgery, data scarcity and imbalance have heavily affected the model accuracy and limited the design and deployment of deep learning-based surgical applications such as surgical instrument segmentation. Considering this, we rethink the surgical instrument segmentation task and propose a one-to-many data generation solution that gets rid of the complicated and expensive pr…

Automatic surgical instrument segmentation of endoscopic images is a crucial building block of many computer-assistance applications for minimally invasive surgery. So far, state-of-the-art approaches completely rely on the availability of a ground-truth supervision signal, obtained via manual annotation, thus expensive to collect at large scale. In this paper, we present FUN-SIS, a Fully-UNsupervised approach for binary Surgical Instrument Segmentation. FUN-SIS trains a per-frame segmentation model on completely unlabelled endoscopic videos, by solely relying on implicit motion information an…

Surgical instrument segmentation is a key component in developing context-aware operating rooms. Existing works on this task heavily rely on the supervision of a large amount of labeled data, which involve laborious and expensive human efforts. In contrast, a more affordable unsupervised approach is developed in this paper. To train our model, we first generate anchors as pseudo labels for instruments and background tissues respectively by fusing coarse handcrafted cues. Then a semantic diffusion loss is proposed to resolve the ambiguity in the generated anchors via the feature correlation bet…

TSMS-SAM2: Multi-scale Temporal Sampling Augmentation and Memory-Splitting Pruning for Promptable Video Object Segmentation and Tracking in Surgical Scenarios
University of Texas Southwestern Medical Center(德克萨斯大学西南医学中心)
Gesture recognition is getting more and more popular due to various application possibilities in human-machine interaction. Existing multi-modal gesture recognition systems take multi-modal data as input to improve accuracy, but such methods require more modality sensors, which will greatly limit their application scenarios. Therefore we propose an end-to-end multi-task learning framework in training 2D convolutional neural networks. The framework can use the depth modality to improve accuracy during training and save costs by using only RGB modality during inference. Our framework is trained …

In this paper, we address the open research problem of surgical gesture recognition using motion cues from video data only. We adapt Optical flow ConvNets initially proposed by Simonyan et al.. While Simonyan uses both RGB frames and dense optical flow, we use only dense optical flow representations as input to emphasize the role of motion in surgical gesture recognition, and present it as a robust alternative to kinematic data. We also overcome one of the limitations of Optical flow ConvNets by initializing our model with cross modality pre-training. A large number of promising studies that a…

Minimally invasive image-guided surgery heavily relies on vision. Deep learning models for surgical video analysis could therefore support visual tasks such as assessing the critical view of safety (CVS) in laparoscopic cholecystectomy (LC), potentially contributing to surgical safety and efficiency. However, the performance, reliability and reproducibility of such models are deeply dependent on the quality of data and annotations used in their development. Here, we present a protocol, checklists, and visual examples to promote consistent annotation of hepatocystic anatomy and CVS criteria. We…

We propose a novel multi-modal and multi-task architecture for simultaneous low level gesture and surgical task classification in Robot Assisted Surgery (RAS) videos. Our end-to-end architecture is based on the principles of a long short-term memory network (LSTM) that jointly learns temporal dynamics on rich representations of visual and motion features, while simultaneously classifying activities of low-level gestures and surgical tasks. Our experimental results show that our approach is superior compared to an ar- chitecture that classifies the gestures and surgical tasks separately on visua…
This work proves that semantic segmentation on minimally invasive surgical instruments can be improved by using training data that has been augmented through domain adaptation. The benefit of this method is twofold. Firstly, it suppresses the need of manually labeling thousands of images by transforming synthetic data into realistic-looking data. To achieve this, a CycleGAN model is used, which transforms a source dataset to approximate the domain distribution of a target dataset. Secondly, this newly generated data with perfect labels is utilized to train a semantic segmentation neural networ…

Many tasks in robot-assisted surgeries (RAS) can be represented by finite-state machines (FSMs), where each state represents either an action (such as picking up a needle) or an observation (such as bleeding). A crucial step towards the automation of such surgical tasks is the temporal perception of the current surgical scene, which requires a real-time estimation of the states in the FSMs. The objective of this work is to estimate the current state of the surgical task based on the actions performed or events occurred as the task progresses. We propose Fusion-KVE, a unified surgical state est…
We apply recurrent neural networks to the task of recognizing surgical activities from robot kinematics. Prior work in this area focuses on recognizing short, low-level activities, or gestures, and has been based on variants of hidden Markov models and conditional random fields. In contrast, we work on recognizing both gestures and longer, higher-level activites, or maneuvers, and we model the mapping from kinematics to gestures/maneuvers with recurrent neural networks. To our knowledge, we are the first to apply recurrent neural networks to this task. Using a single model and a single set of …

Minimally invasive surgery mainly consists of a series of sub-tasks, which can be decomposed into basic gestures or contexts. As a prerequisite of autonomic operation, surgical gesture recognition can assist motion planning and decision-making, and build up context-aware knowledge to improve the surgical robot control quality. In this work, we aim to develop an effective surgical gesture recognition approach with an explainable feature extraction process. A Bidirectional Multi-Layer independently RNN (BML-indRNN) model is proposed in this paper, while spatial feature extraction is implemented …

Automatically recognizing surgical gestures is a crucial step towards a thorough understanding of surgical skill. Possible areas of application include automatic skill assessment, intra-operative monitoring of critical surgical steps, and semi-automation of surgical tasks. Solutions that rely only on the laparoscopic video and do not require additional sensor hardware are especially attractive as they can be implemented at low cost in many scenarios. However, surgical gesture recognition based only on video is a challenging problem that requires effective means to extract both visual and tempo…

Automatic surgical gesture recognition is a prerequisite of intra-operative computer assistance and objective surgical skill assessment. Prior works either require additional sensors to collect kinematics data or have limitations on capturing temporal information from long and untrimmed surgical videos. To tackle these challenges, we propose a novel temporal convolutional architecture to automatically detect and segment surgical gestures with corresponding boundaries only using RGB videos. We devise our method with a symmetric dilation structure bridged by a self-attention module to encode and…
Self-supervised, multi-modal learning has been successful in holistic representation of complex scenarios. This can be useful to consolidate information from multiple modalities which have multiple, versatile uses. Its application in surgical robotics can lead to simultaneously developing a generalised machine understanding of the surgical process and reduce the dependency on quality, expert annotations which are generally difficult to obtain. We develop a self-supervised, multi-modal representation learning paradigm that learns representations for surgical gestures from video and kinematics. …

Kinematic trajectories recorded from surgical robots contain information about surgical gestures and potentially encode cues about surgeon's skill levels. Automatic segmentation of these trajectories into meaningful action units could help to develop new metrics for surgical skill assessment as well as to simplify surgical automation. State-of-the-art methods for action recognition relied on manual labelling of large datasets, which is time consuming and error prone. Unsupervised methods have been developed to overcome these limitations. However, they often rely on tedious parameter tuning and…
Manual labeling of gestures in robot-assisted surgery is labor intensive, prone to errors, and requires expertise or training. We propose a method for automated and explainable generation of gesture transcripts that leverages the abundance of data for image segmentation. Surgical context is detected using segmentation masks by examining the distances and intersections between the tools and objects. Next, context labels are translated into gesture transcripts using knowledge-based Finite State Machine (FSM) and data-driven Long Short Term Memory (LSTM) models. We evaluate the performance of eac…

In recent times there is a growing development of video based applications for surgical purposes. Part of these applications can work offline after the end of the procedure, other applications must react immediately. However, there are cases where the response should be done during the procedure but some delay is acceptable. In the literature, the online-offline performance gap is known. Our goal in this study was to learn the performance-delay trade-off and design an MS-TCN++-based algorithm that can utilize this trade-off. To this aim, we used our open surgery simulation data-set containing …

However, it is very challenging to track the position of the surgical instruments in a surgical scene, and accurate detection & identification of surgical tools is paramount. Deep learning-based semantic segmentation in frames of surgery videos has the potential to facilitate this task.
